摘 要:目的:研究健脾化湿方对脾虚痰湿型肥胖2型糖尿病(T2DM)胰岛素抵抗大鼠脂肪因子的影响。方法:建立脾虚痰湿型肥胖T2DM胰岛素抵抗大鼠模型,将大鼠分为正常对照组、模型组、S模型组、二甲双胍组及健脾化湿方低、中、高剂量组。药物干预4 w,治疗后测量体质量及内脏脂肪;检测大鼠空腹血糖(FBG)、空腹胰岛素(FINS),计算胰岛素敏感指数(ISI)和胰岛素抵抗指数(HOMA-IR);检测血脂水平,包括总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)和游离脂肪酸(FFA);检测脂肪因子水平,包括瘦素(Leptin)、脂联素(Adiponectin)、抵抗素(Resistin)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)。结果:健脾化湿方可显著降低脾虚痰湿型肥胖T2DM胰岛素抵抗大鼠体质量、内脏脂肪、FBG、FINS、HOMA-IR及血清TC、TG、LDL-C、FFA,升高ISI;降低血浆Leptin、Resistin、TNF-α、IL-6,升高Adiponectin,且以高剂量效果最佳(P<0.05或P<0.01)。结论:健脾化湿方可通过良性调节脂肪因子改善脾虚痰湿型肥胖T2DM胰岛素抵抗大鼠糖脂代谢。Objective : To study the effect of spleen-invigorating and dampness-eliminating decoction on adipokines of insulin resist- ance in obese diabetic rats which induced by spleen deficiency and phlegm-dampness. Methods : The model of obese diabetic rats in- duced by spleen deficiency and phlegm-dampness was established. The rats were divided into normal control group, model group, S model group, metformin group and low-, middle-, and high-dose of spleen-invigorating and dampness-eliminating decoction groups. Drug intervention was performed for 4 w. After treatment, the body weight and visceral fat were measured. Fasting blood glucose (FBG)and fasting insulin(FINS) were measured. Insulin sensitivity index(ISI) and insulin resistance index(HOMA-IR) were calculated. The levels of blood lipid,including TC, HDL-C, TG, LDL-C and FFA were measured. The levels of adipokines, including Leptin, Adiponectin, Resis- tin, TNF-α, IL-6 were measured. Results:Spleen-invigorating and dampness-eliminating decoction can significantly reduce weight, vis- ceral fat, FBG, FINS, HOMA-IR, and TC, TG, LDL-C, FFA in serum, and decrease Leptin, Resistin, TNF-α, IL-6 in plasma, and increase IS/and Adiponectin in obese diabetic rats which induced by spleen deficiency and phlegm-dampness. The effect of high-dose group was the best( P 〈 0.05 or P 〈 0. 01 ). Conclusion: Spleen-invigorating and dampness-eliminating decoction can improve the metabolism of glucose and lipid in obese diabetic rats which induced by spleen deficiency and phlegm-dampness through benign regulation of adipokines.
