摘  要:目的 观察硝酸异山梨酯后处理对大鼠在体心肌缺血/再灌注损伤是否具有保护作用,并探讨其作用机制.方法 清洁级雄性Wistar大鼠,32只,10~12周龄.建立大鼠在体心肌缺血/再灌注模型,随机分为假手术组(sham组)、缺血/再灌注组(I/R组)、缺血后处理组(IPostC组)和硝酸异山梨酯后处理组(PPostC组).测定各组大鼠心肌梗死面积,并用Elisa法测定大鼠血清肌钙蛋白Ⅰ(cTnI)、丙二醛(MDA)、超氧化物歧化酶(SOD)水平.结果 ①各组大鼠左室质量和缺血面积未见统计学差异.IPostC组及PPostC组与I/R组相比,心肌梗死面积减小,差异有统计学意义(P<0.05)[(41.015±1.037)%、(38.129±0.726)%比(52.190±1.216)%];而IPostC组与PPostC组之间未见统计学差异(P=0.057).②与sham组相比,Ⅰ/R组、IPostC组和PPostC组血清cTnI的水平显著升高(P.<0.05);与Ⅰ/R组相比,IPostC组和PPostC组的大鼠血清cTnI水平明显降低(P<0.05),IPostC组和PPostC组相比未见统计学差异(P>0.05).③与sham组相比,Ⅰ/R组、IPostC组及PPostC组血清MDA水平显著升高,SOD水平显著降低(P<0.05);与Ⅰ/R组相比,IPostC组及PPostC组血清MDA水平降低,血清SOD水平显著升高(P均<0.05).结论 ①缺血后处理能够减轻大鼠在体心肌缺血/再灌注损伤;②硝酸异山梨酯后处理可以产生与缺血后处理相似的心肌保护效果;③硝酸异山梨酯的心肌保护作用机制可能与减少活性氧的产生及提高心肌的抗氧化(应激)能力有关.Objective To investigate the cardioprotective effect of isosorbide dinitrate (ISDN) postconditioning against rat myocardial ischemia-reperfusion injury in vivo and provide a theoretical basis for the application of the new mode of nitrates postconditioning in clinic.Methods A rat of heart ischemia-reperfusion injury was established.32 male Wistar rats,SPF,10-12 weeks,were randomly divided into four groups as follows:the sham group;the ischemia/reperfusion group (I/R group);ischemic postconditioning group (IPostC group);ISDN group (PPostC group).Infarct size was measured at the end of experiment and the levels of serum of cTnI,MDA and SOD were detected by Elisa.Results (1)There were no significant differences of ischemic sizes between different groups.The myocardial infarct size in IPostC group and PPostC group were significantly decreased compared with Ⅰ/R group(P〈0.05),(41.015±1.037)%,(38.129±0.726),(52.190±1.216)% respectively;and there was no significant difference between IPostC group and PPostC group(P=0.057).(2)Compared with sham group,the levels of cTnI in Ⅰ/R group,IPostC group and PPostC group were significantly increased (P〈0.05).Compared with Ⅰ/R group,the levels of cTnI in IPostC group and PPostC group were significantly decreased(P〈0.05).However,there was no significant difference between IPostC group and PPostC group.(3)Compared with sham group,the levels of MDA in I/R group,IPostC group and PPostC group were significantly increased;the levels of SOD were significantly decreased (P〈0.05).Compared with Ⅰ/R group,IPostC and PPostC could decrease the level of MDA effectively and increase the level of SOD (both P〈0.05).Conclusion (1)Ischemic postconditioning can relieve the ischemia-reperfusion injury.(2)ISDN postconditioning can induce the similar cardioprotective effect as IPostC.(3)The potential mechanisms of cardioprotection of ISDN postconditioning may relate to the improvement of myocardial antioxida
