摘  要:目的探讨HCV核心抗原(HCV-cAg)荧光定量型生物条形码检测(FQ-BCA)在筛查献血员丙肝病毒(HCV)感染中的应用价值。方法收集3 302份献血者的血标本,采用实时-PCR(RT-PCR)及FQ-BCA分别检测血标本的HCV-RNA及HCV-cAg。以RT-PCR检测结果作为金标准,评价FQ-BCA诊断HCV阳性的效能。分析HCV-RNA拷贝数与HCV-cAg阳性检出率的相关性。结果 3 302份血标本中,HCV-RNA阳性共33份,HCV-cAg阳性共32份。FQ-BCA检测的灵敏度为96.97%(32/33),特异度为100.00%(3 269/3 269),符合率为99.97%(3 301/3 302)。HCV-RNA拷贝数与HCV-cAg阳性检出率呈正相关(P<0.05)。结论 HCV-cAg FQ-BCA诊断HCV感染具有极高的检测灵敏度及特异度,适用于献血者的血液筛检。Objective To explore the value of hepatitis C virus core antigen( HCV-cAg) fluorescent quantitative bio-bar-code assay( FQ-BCA) applied to hepatitis C virus( HCV) infection screening in blood donors. Methods A total of 3 302 blood samples were collected from blood donors. Real-time PCR( RT-PCT) and FQ-BCA were used to detect HCV-RNA and HCV-cAg of blood samples respectively. The result of RT-PCR was taken as the golden standard,then the diagnostic efficiency of FQ-BCA for positive HCV was assessed. The correlation of HCV-RNA copy number with positive detection rate of HCV-cAg was analyzed. Results Of 3 302 blood samples,positive HCV-RNA was found in 33 samples and positive HCV-cAg in 32 samples. The sensitivity,specificity and consistency for FQ-BCA detection were 96. 97%( 32/33),100. 00%( 3 269/3 269) and 99. 97%( 3 301/3 302) respectively. The HCV-RNA copy number positively correlated with positive detection rate of HCV-cAg( P 〈 0. 05). Conclusion HCV-cAg FQ-BCA achieves very high sensitivity and specificity for the diagnosis of HCV infection,and can be applied to the blood screening in blood donors.
