急诊经前臂背侧S型切口入路治疗GustiloⅠ,Ⅱ型尺桡骨干骨折的前瞻性研究

摘　　要：目的：探讨急诊经前臂单一背侧S切口入路治疗尺桡骨开放性骨折的方法和疗效.方法：对2011年10月-2015年10月收入院的22例GustiloⅠ,Ⅱ型尺桡骨开放性骨折患者进行前瞻性随机对照研究.其中12例患者行S切口入路治疗,10例行双切口入路治疗.根据手术时间、失血总量、伤口大小,术后第3天长海痛尺评分等比较2组手术创伤情况,根据Grace-Eversmann评分及术后并发症情况比较2组临床疗效.结果：2组随访时间15-18个月,平均随访16个月.S切口组手术时间、失血总量、伤口大小、术后第3天长海痛尺评分均低于双切口组,差异有统计学意义;2组术后并发症发生率分别为S切口组17%,双切口组40%,差异具有统计学意义;2组术后1年Grace-Eversmann评分无统计学差异.结论：急诊经前臂单一背侧S切口入路手术创伤相对较小,患者术后痛苦相对较少,且并发症较少,是一种值得推荐的手术入路.Objective： To investigate the method and effect of emergency treatment of open fracture of the ulnar and radial forearm by single dorsal forearm S incision. Methods： Emergency operation were treated to 22 cases of Gustilo I, II type open fracture of the ulnar and radial. Among them, 12 patients were treated with S incision approach, and 10 patients were treated with double incision approach. We compared the clinical outcomes of the two groups by operation time, total loss of blood, length of incision, pain score, range motion as assessed using a Grace -Eversmann score and the incidence of complications. Results： Patients were followed up for 15 to 18 months（ 16 months in average）. There were significant differences in operation time, total loss of blood, length of incision, pain score and the incidence of complications. There were no significant differences in the Grace - Eversmann score one year after the operation between the two groups. Conclusion： Emergency treatment of Gustilo I, II type of ulnar and radial fractures by S incision in the dorsal forearm was recommended as the preferred method because of the operation was fewer trauma, fewer painful and fewer incidence of complications. So it could be recommended as the suitable method.
