摘  要:目的探讨沿海地区妊娠期妇女的尿碘水平与其甲状腺功能降低的关系。方法通过随机分层抽样法选取某沿海地区妊娠期妇女800例,所有妇女行常规尿碘、甲状腺功能[游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)]检测,依据尿碘水平不同分为A组(<100μg·L-1,464例占58.00%)、B组(100~200μg·L-1,232例占29.00%)和C组(>200μg·L-1,104例占13.00%)。统计分析3组尿碘水平和甲状腺功能情况。结果 B组血清TSH水平明显低于A组和C组,B组血清FT3、FT4水平明显高于A组和C组(均P<0.05),但A组和C组上述指标比较差异无统计学意义(P>0.05);B组甲状腺功能降低总发生率明显低于A组和C组(P<0.05),A组和C组甲状腺功能降低总发生率比较差异无统计学意义(P>0.05)。尿碘水平与妊娠期甲状腺功能降低呈"U"形曲线关系。结论沿海地区妊娠期妇女主要存在碘营养缺乏问题,其尿碘水平与其甲状腺功能降低有关,碘营养缺乏或过量均易增加甲状腺功能降低的发生,提示应合理进行碘营养干预。Objective To investigate the relationship between urine iodine levels and hypothy-roidism in pregnant women in coastal areas.Methods Urine levels of iodine and serum levels of free triiodothyronine(FT3),free thyroxine(FT4)and thyroid stimulating hormone(TSH)were measured in 800 pregnant women selected in coastal areas by stratified random sampling.Accord-ing to the levels of urine iodine,these women were divided into group A(200 μg·L-1 , 104 women,13.00%).Thyroid function was compared among the three groups.Results Com-pared with group A or C,the levels of TSH and incidence of hypothyroidism decreased but the levels of FT3 and FT4 increased in group B(P 〈0.05).No significant differences in serum levels of TSH,FT3 and FT4 and incidence of hypothyroidism were found between group A and group C (P 〉0.05).There was a U-shaped relationship between urine iodine levels and hypothyroidism. Conclusion Iodine deficiency exists in pregnant women in coastal areas,and is correlated with hy-pothyroidism.The lack or overdose of iodine is easy to increase the incidence of hypothyroidism. Therefore,iodine nutrition intervention should be performed in pregnant women in coastal areas.
