摘  要:目的探究经会阴超声在盆腔脏器脱垂(POP)诊断中的应用价值,为POP的超声辅助诊断、量化评估提供参考。方法选取门诊及住院部的POP患者63例(POP组),无明显POP的女性40例(对照组)。经会阴超声观察2组盆底结构,分别在静息期、张力期、缩肛期记录膀胱颈、宫颈、直肠壶腹部的位置及移动度。结果 POP组10例尿道下移、12例真性膀胱膨出、23例子宫脱垂、3例会阴体下移、2例直肠膨出、13例多脏器脱垂,且膀胱颈、宫颈、直肠壶腹部的位置均较对照组低(P<0.05),但移动度比较差异无统计学意义(P>0.05);经临床盆腔脏器量化分期(POP-Q)定量分度Ⅰ度、Ⅱ度脱垂与经会阴超声定量评估的Spearman相关性分析,相关系数分别为0.46、0.83。结论经会阴超声观察盆底结构对诊断POP有重要意义,可为临床诊治提供参考。Objective To explore the value of transperineal ultrasonography in diagnosis of pel-vic organ prolapse(POP),and to provide a reference for ultrasonic-assisted diagnosis and quantita-tive evaluation of POP.Methods Sixty-three patients with POP(POP group)and 40 patients without obvious POP(control group)were selected from outpatient and inpatient departments. Transperineal ultrasonography was performed to observe pelvic floor structure.Moreover,the po-sition and movement of bladder neck,cervix and rectal ampulla were recorded during resting,Val-salva and contraction.Results There were 10 cases of urethral descending,12 cases of bladder bulging,23 cases of uterine prolapse,3 cases of perineal body descending,2 cases of rectal bulging and 13 cases of multiple organ prolapse in POP group.The positions of bladder neck,cervix and rectal ampulla in POP group were lower than those in control group(P 〈0.05).There were no significant differences in movement range between the two groups(P 〉0.05).The Spearman cor-relation coefficients between pelvic organ prolapse quantification(POP-Q)stage Ⅰ and Ⅱ prolapse and transperineal ultrasonographic evaluation were 0.46 and 0.83,respectively.Conclusion The observation of pelvic floor structure by transperineal ultrasonography is of great value in the diag-nosis and treatment of POP.
