乙肝疫苗联合乙肝免疫球蛋白阻断乙肝母婴垂直传播的疗效及影响因素研究

摘　　要：目的探讨乙肝疫苗联合乙肝免疫球蛋白阻断乙肝母婴垂直传播的临床疗效及影响因素。方法选取2012年6月—2014年6月江山市人民医院收治的HBsAg阳性孕妇及其所产新生儿,各80例。新生儿出生后24 h注射乙肝免疫球蛋白(HBIG),并按"0-1-6"程序进行乙肝疫苗接种。检测新生儿的HBsAg和HBs Ab,比较不同喂养方式、分娩方式、产妇"大三阳"/"小三阳"情况及产妇孕前是否按程序接种HBIG新生儿的HBsAg阳性率。结果80例新生儿中,HBsAg阳性6例,阳性率为7.5%;HBs Ab阳性45例,阳性率56.3%。不同喂养方式、分娩方式及产妇孕前是否按程序接种HBIG新生儿的HBsAg阳性率比较,差异无统计学意义(P>0.05);产妇为"大三阳"新生儿的HBsAg阳性率高于产妇为"小三阳"的新生儿(P<0.05)。结论乙肝疫苗联合HBIG能够降低HBV的母婴垂直传播率,孕妇"大三阳"/"小三阳"情况是新生儿HBsAg阳性率的影响因素。Objective To explore the clinical effect of hepatitis B vaccines combined with hepatitis B immunoglobulin on interrupting mother-to-child perpendicular transmission( MTCT) and the influencing factors. Methods A total of 80 pregnant woman suffering from positive HBs Ag were selected in Jiangshan City People' s Hospital of Zhejiang from June 2012 to June 2014,and 80 of their newborns. Neonatal were injected with hepatitis B immunoglobulin with 24 h after birth,and were provided with hepatitis B vaccination in line with " 0-1-6 " procedure. Neonatal were tested for positive rate of HBs Ag and HBs Ab,and were compared for different delivery modes,prenatal immune condition,and the HBs Ag positive rate of " big three positive" and " small three positive" of pregnant women. Results Among all neonatal,6 were tested as positive HBs Ag,and the positive rate was 7. 5%; the positive rate of HBs Ag was 56. 3%. No significant difference was found in HBs Ag positive rate between newborns with different feeding methods, delivery methods and pre pregnancy HBIG inoculation( P > 0. 05). The HBs Ag positive rate of newborns of " big three positive " is higher than that of " small three positive"( P < 0. 05). Conclusion The vertical transmission rate of HBV can be reduced via combined vaccination of hepatitis B vaccine and immunoglobulin. The positive rate of HBs Ag of neonatal birthed by pregnant women with " big three positive" was high.
