负压闭式引流联合自体血胸腔回注治疗复发性气胸的临床研究

摘　　要：目的探讨负压闭式引流基础上行自体血胸膜腔注射治疗复发性气胸的临床疗效。方法选取本院呼吸科2010年6月—2015年12月收治的70例复发性气胸患者,分为观察组39例和对照组31例。观察组患者予负压闭式引流联合100 ml自体血胸腔注射治疗方案,对照组患者予负压闭式引流联合5%葡萄糖+1 g红霉素胸膜腔注射方案。记录两组肺复张前治疗次数、带管天数与并发症及气胸疗效。结果观察组平均治疗次数低于对照组,平均带管天数短于对照组,差异均有统计学意义(P<0.05)。观察组的总有效率率与对照组比较,差异无统计学意义(P>0.05)。随访期间,观察组复发3例,复发率为8.3%(3/36);对照组复发8例,复发率为18.5%(5/27),差异无统计学意义(P>0.05)。结论负压闭式引流基础上行自体血胸膜腔注射较葡萄糖联合红霉素注射治疗复发性气胸能加快肺部复张。Objective To observe and discuss clinical efficacy of closed negative pressure drainage combined with autologous blood pleural reinjection and erythromycin &glucose pleural respectively in the treatment of recurrent pneumothorax.Methods 70 recurrent pneumothorax patients of Jun. 2010 to Dec. 2015 were enrolled in the study and randomly divided into observer group( 39 cases) and control group( 31 cases) according to hospital number,all cases with closed negative pressure drainage therapy,and observer group was added with 100 ml of autologous blood pleural reinjection treatment,control group with pleural injection of 5% glucose & 1 g erythromycin treatment,and therapeutic times before lung restoration,drainage duration and complications of two groups were recorded,and 3 d efficacy of the two groups were determined,besides,pneumothorax recurrence rate in 12 months were followed. Results The observer group with therapeutic times, drainage duration were significantly lower than control group( P < 0. 05). Total effective rate of observer group after treatment for 3 d was higher than control group,there was no statistically significant difference( P > 0. 05). Pneumothorax recurrence rate of observer group 8. 3%( 3/36) during 12 months following up was lower than control group 18. 5%( 5/27),there was no statistically significant difference( P > 0. 05). Conclusion Based on closed negative pressure drainage in treatment of recurrent pneumothorax,autologous blood pleural injection can accelerate the lung restoration.
