切削痂植皮手术联合纳米银敷料及光子治疗仪辅助治疗大面积中重度烧伤的疗效

摘　　要：目的研究大面积中重度烧伤患者给予切削痂植皮手术联合纳米银敷料及光子治疗仪辅助治疗的疗效。方法从2014年10月—2016年10月三峡大学仁和医院烧伤整形科就诊的大面积中重度烧伤患者中选取90例为研究对象,常规组45例行切削痂植皮手术,研究组45例行切削痂植皮手术联合纳米银敷料、光子治疗仪辅助治疗。比较两组治疗后创面愈合时间、发热持续时间、血小板计数下降天数、治疗效果、并发症及死亡情况。结果治疗后研究组创面愈合时间、发热持续时间、血小板计数下降天数均比常规组短;研究组治疗总有效率97.78%(44/45)高于常规组中的84.44%(38/45);研究组的创面侵袭性感染、脓毒症、多器官功能障碍综合征(MODS)、死亡率低于常规组,差异有统计学意义(P<0.05)。结论切削痂植皮手术联合纳米银敷料、光子治疗仪辅助治疗用于大面积中重度烧伤患者可缩短患者恢复时间,降低并发症发生率。Objective To study the effect of scab cutting and skin grafting combined with nano silver dressing and photon therapy instrument in the treatment of large area moderate to severe burns. Methods 90 cases of patients with large area moderate to severe burns in Department of Burns and Plastic Surgery,Renhe Hospital of Three Gorges University from October 2014 to October 2016 were selected the study objects,45 cases of the conventional group received scab cutting and skin grafting,45 cases of the research group received scab cutting and skin grafting combined with nano silver dressing, photon assisted therapy. The wound healing time, fever duration, platelet decline days, treatment effect, complications and mortality were compared between the two groups after treatment. Results After treatment,the wound healing time,duration of fever,platelet decline days of the research group were shorter than those of the conventional group; the research group' s total effective rate of 97. 78%( 44/45) was higher than that of the conventional group 84. 44%( 38/45); the invasive wound infection,sepsis,multiple organ dysfunction syndrome( MODS) and the death rate of the research group were lower than those of the conventional group,the difference had statistical significance( P < 0. 05). Conclusion Scab cutting and skin grafting combined with nano silver dressing and photon therapy instrument can shorten the recovery time and incidence of complications in patients with large area moderate to severe burns.
