摘  要:目的探究鹿瓜多肽联合唑来膦酸治疗糖皮质激素性骨质疏松症的近、远期疗效以及对骨密度(BMD)的影响。方法选择2013年4月—2014年3月湖北文理学院附属医院骨科治疗的80例糖皮质激素性骨质疏松症患者,采用随机数表法将患者分为观察组(n=40)和对照组(n=40),对照组采用唑来膦酸治疗,观察组在对照组基础上加用鹿瓜多肽治疗。比较两组临床疗效,并检查治疗前、治疗1年、2年、3年后第1~4腰椎、左侧股骨颈、粗隆间、大转子、Ward'S三角BMD,同时采用疼痛视觉模拟(VAS)评分评价疼痛程度。结果观察组治疗总有效率为90.0%(36/40),高于对照组75.0%(30/40),差异有统计学意义(P<0.05)。两组治疗1年、2年、3年后各部位BMD高于治疗前,治疗1年后观察组各部位BMD高于对照组,差异有统计学意义(P<0.05)。两组治疗1年、2年、3年后VAS评分低于治疗前,治疗1年后观察组VAS评分低于对照组,差异有统计学意义(P<0.05)。结论鹿瓜多肽联合唑来膦酸治疗糖皮质激素性骨质疏松症具有更为显著的临床疗效,近期缓解疼痛、提高BMD较为明显。Objective To study the long-term effect of cervus and cucumis polypeptide combined with zoledronic acid in treating glucocorticoids-induced osteoporosis and the effect on bone mineral density( BMD). Methods 80 patients with glucocorticoids-induced osteoporosis treated in Orthopaedics of Affiliated Hospital of Hubei College of Arts and Sciences from April 2013 to March 2014 were selected,using a random number table method were divided into the observation group( n =40) and control group( n = 40). Retrospective analysis was conducted on case data of patients who meet criteria. Control group was treated with zoledronic acid,while the observation group was treated with cervus and cucumis polypeptide combined with zoledronic acid. Two groups were compared for clinical efficacy, and were examined for BMD at lumbar vertebra 1 ~ 4,femeralneck,intertrochanteric,greater trochanter and Ward'S triangle in before treatment and 1 year,2 years and 3 years after treatment,meanwhile,VAS pain score was adopted to evaluate symptom of pain in before treatment and 1 year,2 years and 3 years after treatment. Results Total treatment efficacy of observation group was 90. 0%( 36/40), which was significantly higher than control group [75. 0%( 30/40) ],the difference was statistically significant( P < 0. 05). The BMD of both groups in 1 year,2 years and 3 years after treatment was distinctly higher than that before treatment,in 1 year after treatment,BMD of observation group was higher than control group,the difference was statistically significant( P < 0. 05). The VAS score of both groups in 1 year,2 years and 3 years after treatment was distinctly lower than that before treatment,in 1 year after treatment,VAS score of observation group was lower than control group,the difference was statistically significant( P < 0. 05). Conclusion Cervus and cucumis polypeptide combined with zoledronic acid has excellent clinical efficacy in treating glucocorticoids-induced osteoporosis,which can also relieve pain and dramatically improve BMD.
