液体复苏后24 h胃管注入大黄附子汤联合西医基础治疗对急诊ICU脓毒症患者胃肠功能改善的作用

摘　　要：目的研究大黄附子汤对急诊ICU脓毒症患者的胃肠功能疗效。方法选取2012年5月—2015年5月河南省商丘市第一人民医院脓毒症患者72例,根据随机数表法将患者分为试验组和对照组,入住ICU病房后均给予急性胃肠功能损伤(AGI)分级评估,对照组给予常规治疗,试验组在对照组基础上注入大黄附子汤。观察治疗前后AGI分级评估;ICU病房住院时间、住院总时间、机械通气时间、使用血管活性药物或正性肌力药物时间、新发生感染率、28 d全因病死率的影响因素。结果两组治疗前AGI分级情况比较,差异无统计学意义(P>0.05),治疗后试验组AGI分级情况比较,差异有统计学意义(P<0.05)。两组的ICU病房住院时间和住院总时间、使用血管活性药物或正性肌力药物时间比较,差异无统计学意义(P>0.05);试验组机械通气时间、新发生感染率均低于对照组,差异有统计学意义(P<0.05);大黄附子汤对28 d全因病死率无影响。结论大黄附子汤有效改善胃肠功能。Objective To study the effect of Dahuang Fuzi Decoction on gastrointestinal function in patients with sepsis in ICU. Methods 72 cases with sepsis in the Shangqiu First People's Hospital of Henan Province from May 2012 to May 2015 were selected. According to the random number table method, the patients were divided into experimental group and control group. The patients in the control group was given with conventional treatment and the patients in the experimental group were gievn with Dahuang Fuzi Decoction based on the treatment of the control group. The AGI grade were evaluated before and after treatment. The ICU ward hospitalization time,hospitalization time,mechanical ventilation time and the use of vasoactive drugs or inotropic drugs and the new infection rate were compared between two group. The factors influencing the mortality were investigated by cox regression. Results There was no significant difference between the two groups about the AGI grade before treatment( P > 0. 05). After treatment,the AGI grade in the experimental group was statistically different with that in the control group( P < 0. 05). The ICU ward hospitalization time,total hospitalization time,the use of vasoactive drugs or positive inotropic drugs time were not statistically different between two groups( P > 0. 05). The mechanical ventilation time and new infection rate in the experiment were lower than that in the control group and the difference were statistically significant( P <0. 05). Dahuang Fuzi Decoction had no effect on the mortality of the patients. Conclusion Dahuang Fuzi Decoction can effectively improve gastrointestinal function.
