维持性血液透析患者肺癌肿瘤标志物参考值的建立

摘　　要：目的确定维持性血液透析透患者癌胚抗原(CEA)、鳞状细胞癌抗原(SCC)、角蛋白19片段(Cyfra21-1),神经元特异性烯醇化酶(NSE)等肺癌肿瘤标志物的特殊人群参考值范围。方法选择我院2011年至2014年血液净化科维持性血液透析6个月以上的终末期肾病患者共320例,其中男158例,女162例,健康查体者372例,男189例,女183例,采用电化学发光法检测角蛋白19片段(Cyfra21-1),神经元特异性烯醇化酶(NSE),采用化学发光法检测癌胚抗原(CEA)、鳞状细胞癌抗原(SCC),建立维持性血液透析患者肺癌肿瘤标志物的正常参考值。结果维持性透析患者肿瘤标志物检测值和阳性率均高于正常对照组。维持性血液透析患者95分位检测值分别为:CEA 7.94ng/ml、SCC 7.2ng/ml、Cyfra21-1 11.86ng/ml、NSE 34.69ng/ml。结论维持性血透患者特殊人群肺癌肿瘤标志物参考范围如下:CEA≤7.94ng/ml,SCC≤7.2ng/ml,Cyfra21-1≤11.86ng/ml,NSE≤34.69ng/ml。Objective To determine the reference intervals for lung tumor markers in maintenance dialysis patients. Methods We recruited 320 maintenance dialysis patients,including 158 males and 162 females. A reference population was recruited to be comprised of 372 healthy adult volunteers, including 189 males and 183 females. Chemiluminescence method was applied to detect the serum levels of CEA and SCC. Electrochemiluminescence method was applied to detect the serum levels of Cyfra21-1 and NSE. A reference interval was determined using the non-parametric method. Results The concentration and positive rate of tumor markers in maintenance dialysis patients were higher than those healthy adult volunteers. The upper 95th percentile for CEA, SCC, Cyfra21-1, and NSE was determined to be 7.94 ng/ml, 7.2 ng/ml,11. 86 ng/ml,34.69 ng/ml,respectively. Conclusion Based on this study,The reference interval for CEA,SCC, Cyfra21-1, and NSE was determined to be 7.94 ng/ml, 7.2 ng/ml, 11.86 ng/ml, 34.69 ng/ml, respectively.
