加巴喷丁胶囊联合氨酚曲马多片治疗带状疱疹后遗神经痛的临床效果观察

摘　　要：目的观察加巴喷丁胶囊联合氨酚曲马多片治疗带状疱疹后遗神经痛(PHN)的临床效果。方法将医院2014年3月-2017年3月疼痛科收治的PHN患者60例,随机分为观察组和对照组,每组30例。对照组单纯给予加巴喷丁治疗,观察组患者在对照组基础上联合使用氨酚曲马多片治疗。治疗结束后,评价2组患者的治疗效果,随访2 d,采用视觉疼痛评分(VAS)对2组患者治疗前后疼痛改善情况进行比较。结果观察组治疗总有效率为83.33%高于对照组的70.00%,差异有统计学意义(P<0.05)。随访2 d 2组患者治疗后VAS评分较治疗前均有显著改善,且观察组显著优于对照组,差异有统计学意义(P<0.05)。治疗过程中,观察组出现眩晕1例、恶心1例;对照组出现疲劳1例,头痛1例,2组不良反应发生率均为6.67%,2组比较无明显差异(P>0.05)。结论加巴喷丁胶囊联合氨酚曲马多片治疗PHN的效果显著优于单纯使用加巴喷丁,不但能显著改善患者症状,更能缓解疼痛,有助于预后,值得推广。Objective To investigate the clinical effect of gabapentin capsules combined with leflum tramadol in the treatment of postherpetic neuralgia（ PHN）. Methods 60 patients with PHN,treated in pain department in the hospital from March 2014 to March 2017,were randomly divided into observation group and control group,30 cases in each group. Control group was treated with gabapentin alone,and the patients in observation group were treated with paracetamol on the basis of the control group. After treatment,the therapeutic effect of two groups was evaluated and the visual acuity score（ VAS） was used to compare the improvement of pain before and after treatment for 2 days. Results The total effective rate was 83. 33% in observation group after treatment higher than 70. 00% in control group,the difference was statistically significant（ P 〈 0. 05）. The VAS scores of two groups were significantly improved compared with those before treatment,and observation group was significantly better than control group,the difference was statistically significant（ P 〈 0. 05）. In the course of treatment,there was 1 case of vertigo and 1 case of nausea in the observation group. 1 case of fatigue and 1 case of headache in control group. The incidence of adverse reactions in two groups was 6. 67%,and there was no significant difference between the two groups（ P 〉0. 05）. Conclusion The effect of gabapentin combined with lecitholone on the treatment of postherpetic neuralgia is superior to that of gabapentin alone. It can not only improve the symptoms of the patients,but also alleviate the pain and help the prognosis.
