经胸大肌下囊袋植入埋藏式心脏转复除颤器的临床观察

摘　　要：目的观察经胸大肌下囊袋植入埋藏式心脏转复除颤器(ICD)的临床效果。方法8例胸壁组织薄弱患者经胸大肌下囊袋植入ICD或心脏同步化治疗除颤器(CRT-D)。观察术后囊袋血肿、破溃及感染等并发症发生情况。结果 8例中,因囊袋内渗血较多放置引流装置3例,均于48h内拔除引流。所有患者手术切口均愈合良好,随访1~42个月未发现囊袋血肿、囊袋皮肤压迫性坏死、破溃或感染等情况。结论经胸大肌下囊袋植入ICD或CRT-D切实可行,可作为相对消瘦患者的合适选择。Objective To observe the outcomes of implanting cardioverter defibrillator （ICD ） via a pocket beneath the major pectoral muscle .Methods The ICD or cardiac resynchronization therapy defibrillator（CRT-D） were implanted in 8 patients with thin chest wall .The postoperative complications including pocket hematoma ,erosion and infection were recorded .Results The drainage of pocket was performed in 3 cases due to blood oozing in pocket ,which was withdrawn at 48 hours later .The patients were followed up for 1-42 months .The pocket was well healed without pocket hematoma ,skin oppressive necrosis ,erosion or infection .Conclusion Implanting ICD or CRT-D via a pocket beneath the major pectoral muscle is feasible ,which is a suitable choice in the patients with thin chest wall .
