盐酸羟考酮超前镇痛抑制腹腔镜辅助下胃癌根治术后急性疼痛的临床研究

摘　　要：目的:探讨盐酸羟考酮超前镇痛对腹腔镜胃癌根治术后镇痛效果的影响。方法:选取某院2013年6月~2015年11月之间行腹腔镜辅助下胃癌根治术治疗的136例患者,ASA I或Ⅱ级,年龄28~74岁,随机均分为超前镇痛组(A组68例)和术后镇痛组(B组68例),A组在术前30min静推盐酸羟考酮注射液0.10mg/kg,B组在手术结束时静脉推注盐酸羟考酮注射液0.10mg/kg。手术结束后1、2、4、8、12、24h对患者术后的疼痛程度采用视觉模拟评分法(VAS)进行评分,并观察术后患者有无恶心、呕吐、呼吸抑制等不良反应的发生情况。结果:A组在术后1、2、4、8、12、24h的VAS评分明显低于B组,P<0.05;两组在恶心、呕吐、呼吸抑制等并发症发生率方面差异无统计学意义,P>0.05。结论:盐酸羟考酮超前镇痛能有效减轻腹腔镜辅助下胃癌根治术后急性疼痛,且不增加术后并发症的发生率。Objective： To investigate the preemptive analgesic effects of oxyeodone hydrochloride administration on postoperative pain in patients undergoing laparoscope-assisted radical gastrectomy. Methods： 136 ASA Ⅰ or Ⅱ patients aged 28474 yearS, from Jun 2013 to Nov 2015 in our hospital, undergoing laparoscopeassisted radical gastrectomy were divided into two groups randomly. The patients in the preemptive analgesic group （Group A） were intravenously injected with oxycodone hydrochloride 0.10mg/kg 30 minutes before surgery. The patients in the postoperative analgesic group （Group B） were injected with oxycodone hydrochloride 0.10 mg/kg after the surgery. The analgesic effects was evaluated by visual analog scale （VAS） at 1, 2,4,8,12,24h at the end of surgery. The occurrence rate of adverse reactions including nausea, vomit and respiratory depression was recorded. Results. Compared with group B, VAS of group A were lower at 1,2,4,8, 12,24h at the end of surgery postoperatively（P〈0.05）. There was no statistical difference in nausea ,vomit , respiratory depression between two group （P 〉 0.05）. Conclusion ： Preemptive analgesic effects of oxycodone hydrochloride administration on postoperative pain is effective and safe in patients undergoing laparoscope-assisted radical gastrectomy, which does not increase the incidence of postoperative complication.
