负压吸引联合硬化剂治疗微囊型淋巴管畸形

摘　　要：目的:探讨负压吸引联合硬化剂治疗微囊型淋巴管畸形的临床效果。方法:选取2016年8月至2017年6月于笔者科室就诊的微囊型淋巴管畸形患儿8例,采用术前MRI、术中彩超引导,对微囊型淋巴管畸形应用负压抽吸针抽吸淋巴液,打通各囊间隔,再于瘤体中注入无水乙醇及聚多卡醇泡沫等硬化剂,适当加压包扎治疗微囊型淋巴管畸形。结果:全部病例随访3~10个月,6例患儿瘤体完全消失;1例治疗后瘤体明显缩小,未完全消失,继续随诊,未有瘤体增大及局部压迫症状;1例治疗后出现囊内出血,经第二次硬化治疗后瘤体明显缩小。全部病例均未见皮肤坏死等严重并发症。结论:负压吸引联合硬化剂治疗微囊型淋巴管畸形,疗效确切,副作用小,治疗次数少,值得临床推广应用。Objective To investigate the therapeutic effect of combined Pressure suction and sclerotherapy on the microcystic lymphatic malformation. Methods From August 2016 to June 2017, 8 children with microcystic lymphatic malformation were enrolled in our department. Using preoperative magnetic resonance imaging（MRI）, intraoperative high-frequency ultrasound to exceed, lymphatic malformation of the sacs of micro capsule type application of negative pressure suction needle suction pump,through the sac interval,again in tumors had injection absolute ethyl alcohol and polidocanol foam, appropriate compression bandage treat the type of micro capsule lymphatic malformation. Results All cases were followed up for 3-10 months, tumor disappeared completely in 6 cases.In one case, the tumor was obviously reduced, not completely disappeared, without tumor enlargement and local compression symptoms. Intracapsular hemorrhage occurred in one case,the tumor volume decreased signifcantly after second sclerosis treatment. No serious complications such as skin necrosis were observed in all cases. Conclusion Pressure suction combined sclerotherapy with microcystic lymphatic malformation therapy, curative effect, side effect is small, fewer treatment, worthy of clinical popularization and application.
