达芬奇内窥镜辅助前列腺根治术与腹腔镜根治术术后医院感染分析

摘　　要：目的对比分析达芬奇内窥镜辅助前列腺根治术与腹腔镜根治术术后医院感染情况。方法选取医院2015年6月-2016年6月行前列腺根治术患者86例为试验组,采用达芬奇内窥镜辅助前列腺根治术治疗;2014年6月-2015年5月在医院行腹腔镜下前列腺根治术患者98例为对照组,比较两组术后医院感染发生情况。结果两组在术后留置尿管时间上并无明显差异,但试验组手术时间、术中出血量、术后住院时间均明显少于对照组,差异有统计学意义(P<0.05);试验组患者发生医院感染7例,感染率为8.13%,对照组感染18例感染率为18.36%,两组比较差异有统计学意义(P<0.05);试验组感染患者共培养出病原菌18株,对照组检出29株,均以革兰阴性菌为主,分别占72.22%、75.86%。结论达芬奇内窥镜辅助前列腺根治术相比腹腔镜根治术,发生术后医院感染概率更低,具有更好的临床应用价值。OBJECTIVE To compare the prevalence of postoperative nosocomial infections between the patients un-dergoing radical prostatectomy assisted by DaVinci endoscope and the patients undergoing laparoscopic radical re-section.METHODS Totally 86 patients who underwent radical prostatectomy in the hospital from Jun 2015 to Jun 2016 were chosen as the observation group and were treated with radical prostatectomy assisted by DaVinci endo-scope,and 98 patients who underwent laparoscopic radical prostatectomy in the hospital from Jun 2014 to May 2015 were set as the control group.The prevalence rate of postoperative nosocomial infection was compared be-tween the two groups.RESULTS There was no significant difference in the postoperative catheter indwelling time between the two groups,however,the operation duration and postoperative length of hospital stay of the observa-tion group were significantly shorter than those of the control group,the intraoperative blood loss volume of the observation group was significantly less than that of the control group（P〈0.05）.Totally 7 patients had nosoco-mial infections in the observation group,with the infection rate 8.13%,and 18 patients had nosocomial infections in the control group,with the infection rate 18.36%,there was significant difference between the two groups（P〈0.05）.Totally 18 strains of pathogens were isolated from the patients with the infections in the observation group,of which 72.22% were gram-negative bacteria;29 strains of pathogens were isolated from the patients with the infection in the control group,of which 75.86% were gram-negative bacteria.CONCLUSION The inci-dence of postoperative nosocomial infections is significantly lower in the patients undergoing the radical prostatec-tomy assisted by DaVinci endoscope than in the patients undergoing the laparoscopic radical resection,and it has higher clinical application value.
