血清β-HCG与h-CRP和CRP对胎膜早破孕妇宫内感染的预测价值分析

摘　　要：目的探究血清β-HCG、超敏C-反应蛋白(h-CRP)和C-反应蛋白(CRP)水平对胎膜早破患者宫内感染的预测价值,以期提升胎膜早破孕妇的预后质量。方法选择2015年2月-2016年2月医院收治的42例胎膜早破孕妇作为研究对象,检测所有孕妇血清β-HCG、h-CRP和CRP水平,以胎膜病理检测结果为金标准,计算血清β-HCG、h-CRP和CRP诊断宫内感染的敏感性、特异性、准确度。结果受试者工作曲线结果显示,血清β-HCG、h-CRP、CRP宫内感染的临界值分别为:2.09ng/ml,3.69mg/L,0.11g/L;血清β-HCG+h-CRP、β-HCG+h-CRP+CRP的灵敏度、特异度和准确度分别为:80.00%、91.67%、83.33%、83.33%、91.67%、85.71%,明显优于其它组合或者单一指标预测。结论血清β-HCG+h-CRP+CRP三者联合预测胎膜早破孕妇是否并发宫内感染具有较高的临床价值,为临床治疗措施选择提供重要信息,促进胎膜早破孕妇母儿安全具有重要临床价值。OBJECTIVE To explore the clinical value of serum beta-HCG,high-sensitivity C-reactive protein（h-CRP）and C-reactive protein（CRP）levels in prediction of intrauterine infection in patients with premature rupture of membranes（PROM）so as to improve the quality of prognosis of the pregnant women with PROM.METHODS A total of 42 pregnant women with PROM who were treated in the hospital from Feb 2015 to Feb 2016 were re-cruited as the study objects,the levels of serumβ-HCG,h-CRP and CRP of all the pregnant women were deter-mined,the result of pathological examination of membranes was set as the gold standard,and the sensitivity,spe-cificity and accuracy of the serum β-HCG,h-CRP and CRP in diagnosis of the intrauterine infection were calculat-ed.RESULTS The result of receiver-operating-curve indicated that the critical values of the serum β-HCG,h-CRP and CRP in diagnosis of the intrauterine infection were respectively 2.09ng/ml,3.69mg/L and 0.11g/L.The sen-sitivity,specificity and accuracy of serum β-HCG plus h-CRP,β-HCG plus h-CRP plus CRP were respectively 80. 00%,91.67%,83.33%,83.33%,91.67% and 85.71%,significantly higher than those of other combinations or single index.CONCLUSION The serum β-HCG plus h-CRP plus CRP has high clinical value in prediction of in-trauterine infection in the pregnant women with PROM and has significant clinical value in promoting the mother-infant safety,and it can provide important information for the clinical treatment.
