抗乙酰胆碱3受体抗体与原发性干燥综合征相关性研究

摘　　要：目的探讨抗毒蕈碱3型乙酰胆碱受体抗体(a-M3R)在原发性干燥综合征(p SS)中的临床意义。方法通过酶联免疫吸附试验(ELISA)检测p SS组(42例)和疾病对照组(系统性红斑狼疮及类风湿关节炎患者18例)、健康对照组(20例)的抗M3R抗体阳性率;并将p SS组分为抗M3R抗体阳性组和阴性组,分析p SS组中抗M3R抗体阳性患者与阴性患者在临床及免疫学等指标间的差异与关系。结果抗M3R抗体在p SS患者中的阳性率为71.43%,疾病对照组的阳性率为16.67%,健康对照组的阳性率为15.00%,p SS患者阳性率高于疾病对照组和健康对照组(P<0.05)。p SS组抗M3R抗体阳性患者Ig G、Ig A、RF值及抗SSB抗体阳性例数较阴性患者明显增高。随着唇腺病理分级增高,抗M3R OD值随之升高。p SS抗M3R阳性患者血液系统受累较阴性患者明显升高。结论抗M3R抗体对p SS有一定的辅助诊断价值;抗M3R抗体阳性的p SS患者易合并血液系统病变,唇腺组织机构破坏较重。Objective To investigate the anti - muscarinic 3 acetylholine receptor antibody （ a - M3R） in primary Sjgren＇ s syndrome （pSS） clinical significance, and the relationship with organ involvement and with labial gland pathology classification. Methods The anti M3R antibody was detected by ELISA in pSS group （42 cases） and control group （ 18 cases of systemic lupus erythematosus and rheumatoid arthritis） and healthy volunteers （20 cases） ;and the pSS group was divided into Anti M3 R antibody positive group and negative group, and analyzed the differences and relationship in clinical and immunological indexes. Results The positive rate of anti M3R antibody in pSS patients was significantly higher than that in the disease control group and the healthy control group （71.43%, 16.67% and 15% ,P 〈 0.05）. In the pSS group, the number of IgG, IgA, RF and anti SSB antibodies in patients with positive anti M3R antibody was significantly higher than that in the negative patients. With the pathological grading of the labial glands increased, the anti M3ROD value increased. The blood system irwolvement in pSS positive M3R patients was significantly higher than that in the negative patients. Conclusion The detection of serum anti M3R antibody has certain auxiliary diagnostic value for pSS. The patients with anti M3R antibody positive pSS are prone to complicated with hematological diseases; the patients with high concentration of anti M3R antibodies have severe labial gland destruction in pSS patients.
