浙江省杭州市余杭区40岁以上人群心血管风险评估

摘　　要：目的了解余杭区40岁以上人群心血管疾病相关危险因素的流行特征,以及在未来10年发生心血管疾病的发病风险。方法采用多阶段整群抽样方法,抽取余杭区40岁以上居民6 470名,进行心血管相关危险因素流行病学调查,并利用《心血管风险评估和管理袖珍指南》风险预测图对研究对象进行心血管风险预测。结果研究对象中吸烟、有糖尿病史、血压异常以及血脂异常暴露率分别为20.29%、6.72%、36.48%、22.84%;人群中具有≥1种危险因素的人群占61.81%,≥2种危险因素的人群占20.48%;人群中未来十年发生心血管疾病风险≥10%的比例达到了10.74%,男性较女性、年龄高者较年纪轻者发生心血管疾病的风险概率高(P<0.01);年龄,SBP值、血总胆固醇,男性,吸烟以及有糖尿病都是心血管疾病发生的危险因素,其OR值分别为1.09、1.09、1.94、2.59、2.22及5.56,其中有糖尿病的人群在未来十年发生心血管疾病的危险性最大。结论余杭区40岁以上人群心血管危险因素水平较高,未来十年发生心血管疾病的风险较大,提示应加强对心血管疾病危险因素的综合干预,尤其是聚集多个危险因素的人群。Objective To understand the epidemiological features of risk factors associated with cardiovascular disease （CVD） and the attack risk in the next 10 years of above 40 years old people in Yu hang District. Methods The multi-stage cluster sampling was used to select 6 470 residents which were above 40 years old. And using the pocket guidelines for assessment and management of cardiovascular risk to predict the attack risk in the next 10 years. Results The prevalence rate of smoking, diabetes, hypertension and dyslipidemia were 20.29%, 6.72%, 36.48%, 22.84% ; The rate of more than one risk factor in the people was 61.81%,more than two risk factors was 20.48%; The proportion of the attack risk which was more than 10% in the next 10 years of above 40 years old people was 10.74%; The morbidity probability of male and older were higher than the females and younger（P〈0.01 ） ; The odds ratio of age, SBP, blood total cholesterol, sex, smoking and diabetes were 1.09, 1.09, 1.94, 2.59, 2.22 and 5.56. Conclusion The levels of cardiovascular disease of risk factors in Yuhang District is high and the risk level of CVD is higher in the people of above 40 years old. The comprehensive intervention on CVD risk factors should be enhanced, especially the people who gathered muhiDle risk factors.
