摘  要:目的通过检测血管相关因子富半胱氨酸蛋白61(Cyr61)、结缔组织生长因子(CTGF)、血管内皮生长因子(VEGF)和CD105标记的微血管密度(MVD)在PM、DM患者肌肉组织中的表达,探讨Cyr61、CTGF、VEGF和CD105对PM/DM血管病变过程的作用及意义。方法采用免疫组织化学方法测定10例PM患者、10例DM患者及20名对照组的肌肉石蜡组织连续切片中Cyr61、CTGF、VEGF阳性细胞数和CD105标记的MVD;数据采用SPSS统计软件进行分析。采用Fisher确切概率分析和Spearman相关分析。结果Cyr61、CTGF、VEGF在DM组和PM组患者肌肉组织中的阳性表达率分别为90%、70%、90%、80%、80%、70%,与对照组(阳性表达率分别为5%、10%、5%)比较差异均有统计学意义(P〈0.01);DM组和PM组患者的肌肉组织中可见CD105标记的微血管,且DM组和PM组的MVD均高于对照组,差异均有统计学意义(F=8.103,P=0.001);DM组和PM组患者肌肉组织中的Cyr61、CTGF、VEGF蛋白表达水平与MVD变化均呈正相关(P〈0.05)。结论PM/DM患者肌肉组织中可能都有新生血管的形成;Cyr61、CTGF、VEGF可能参与了PM/DM肌肉组织中新生血管的形成过程;提示在PM/DM的免疫发病机制中微血管病变起着至关重要的作用。ObjectiveBy detecting vascular cysteine-rich 61(Cyr61) related factor, connective tissue growth factor(CTGF), vascular endothelial growth factor(VEGF) and CD105 markers of microvascular density (MVD) of muscle tissue in patients with PM/DM, the role and significance of the expression of Cyr61, CTGF, VEGF and CD105 in the process of vascular lesions of dermatomyosits(DM) and polymyosits(PM) were discussed.MethodsThe expression of Cyr61, CTGF, VEGF and CD105 markers of micro vascular density (MVD) were detected in 10 cases of DM, 10 cases of PM and 20 controls by using immunohistochemical Envision two step method. Data were analyzed using Statistical Product and Service Solutions (SPSS) statistical software. Fisher's exact probability analysis and Spearman correlation analysis were conducted.ResultsCompared with the control group, Cyr61, CTGF, VEGF positive expression rate in muscle tissue of patients with DM and PM group were significantly different (P〈0.01), the positive expression rates of Cyr61, CTGF, VEGF in DM group and PM group were 90%, 70%, 90%, 80%, 80%, 70%, and the control group (5%, 10%, 5%) respectively. In the muscle tissue of patients with DM and PM group, CD105 markers of capillaries could be seen, and MVD in DM and PM group were higher than that in the control group, the difference was statistically significant (F=8.103, P=0.001). Cyr61, CTGF and VEGF protein expression levels in muscle tissue of patients with DM and PM were positively correlated with MVD.ConclusionThe muscle tissue of PM/DM patients may have new blood vessels formation. Cyr61, CTGF, VEGF may be involved in the formation of new blood vessels in the PM/DM muscle tissue. The results of this study suggest that microvascular lesion plays an important role in the immune pathogenesis of inflammatory myopathy such as PM/DM.
