天突穴针刺联合西药治疗咽喉反流性疾病的临床观察

摘　　要：目的观察天突穴针刺联合奥美拉唑治疗咽喉反流性疾病的临床疗效。方法将202例咽喉反流性疾病患者随机分为治疗组(102例)和对照组(100例)。对照组予奥美拉唑肠溶胶囊口服,治疗组在对照组治疗措施基础上加用天突穴针刺治疗。两组疗程均为2个月,观察临床疗效,比较咽喉反流症状指数量表(RSI)、咽喉反流体征评分量表(RFS)各分项评分及总分的变化情况。结果 (1)治疗组、对照组临床总有效率分别为78.43%、63.00%;组间临床疗效比较,治疗组明显优于对照组(P<0.05)。(2)组间治疗后比较,RSI中的持续清嗓、饭后或躺下后咳嗽、烦人的咳嗽、咽喉异物感评分及总分差异有统计学意义,治疗组明显优于对照组(P<0.05)。组间治疗后比较,RFS中的红斑/充血、声带水肿、弥漫性喉水肿、后连合增生评分及总分差异有统计学意义,治疗组明显优于对照组(P<0.05)。结论天突穴针刺联合奥美拉唑口服治疗咽喉反流性疾病的疗效满意。Objective To observe the clinical efficacy of acupuncture on Tiantu acupoint （CV22） combined with omeprazole in the treatment of laryngopharyngeal reflux disease. Methods 202 patients of laryngopharyngeal reflux disease were randomly divided into the treatment group （ 102 cases） and control group （ 100 cases）. The control group was orally treated with omeprazole enteric-coated capsule, and the treatment group was treated with acupuncture on Tiantu acupoint based on the treatment for the control group, with a course of 2 months. The clinical efficacy was observed, and the changes on the single item score and total score in reflux symptom index （RSI） scale and reflux finding score （RFS） scale were compared. Results ①The clinical total effective rates were 78.43% in the treatment group and 63.00% in the control group. The clinical efficacy of the treatment group was significantly better than that of the control group（ P 〈 0.05 ）. ②Mter treatment, there were statistically significant differences on the RSI total score and single item score of persistent threat clearing, cough after meals or lying down, disturbing cough and foreign body sensation in throat between the two groups, and the improvement on the symptoms in the treatment group was obviously better than that in the control group （ P 〈 0.05 ）. After treatment, there were statistically significant differences on the RFS total score and single item score of erythema/hyperemia, edema in vocal cords, diffuse laryngeal edema and posterior commissure hyperplasia between the two groups, and the improvement on the signs in the treatment group was obviously better than that in the control group （P 〈 0.05 ）. Conclusion Acupuncture on Tiantu acupoint combined with omeprazole by oral administration shows good efficacy in the treatment of laryngopharyngeal reflux disease.
