益气养阴清热法治疗湿热下注型高位肛瘘术后创面肿痛临床观察

摘　　要：目的观察益气养阴清热法治疗湿热下注型高位肛瘘术后创面肿痛的疗效。方法将78例湿热下注型高位肛瘘术后患者随机分成观察组与对照组(各39例),分别予益气养阴清热法和清热渗湿法治疗,疗程4周;观察患者证候疗效及创面肿痛积分变化情况。结果 (1)观察组的证候疗效优于对照组(P<0.05),黄腻苔的改善尤其明显(P<0.01)。(2)观察组创面疼痛、渗出和水肿积分均低于对照组(P<0.05,P<0.01)。结论益气养阴清热法较单纯清热渗湿法能更有效减轻湿热下注型高位肛瘘术后临床症状和创面肿痛。Objective To observe the efficacy of therapy of benefiting qi and nourishing yin and clearing heat in the treatment of postoperative wound swelling and pain of high anal fistula with syndrome of dampness-heat diffused downward. Methods 78 patients of high anal fistula with syndrome of dampness-heat diffused downward who had received the operation were randomly divided into the observation group and control group ,39 cases in each group. The observation group was treated with therapy of benefiting qi and nourishing yin and clearing heat, and the control group was treated with therapy of clearing heat and eliminating dampness, with a course of 4 weeks. The changes on the syndrome efficacy and scores of wound swelling and pain in patients were observed. Results ①The syndrome efficacy of the observation group was better than that of the control group（P 〈0.05） ,especially the improvement on yellow greasy fur（P 〈0.01 ）. ②The scores of wound pain, exudation and edema in the observation group were all lower than those in the control group （ P 〈 0.05,P 〈 0.01 ）. Conclusion Compared with the single therapy of clearing heat and eliminating dampness, the therapy of benefiting qi and nourishing yin and cleating heat can more effectively alleviate the postoperative clinical symptoms and wound swelling and pain of high anal fistula with syndrome of dampness-heat diffused downward.
