探讨仫佬族慢性心力衰竭与N末端B型钠尿肽前体的关系

摘　　要：目的探讨仫佬族慢性心力衰竭(CHF)与N末端B型钠尿肽前体(NT-pro BNP)的关系;方法选仫佬族慢性心力衰竭和汉族慢性心力衰竭患者各30例为观察组,同期仫佬族健康体检者20例为对照组。采用热景UPT-3A上转发光免疫分析法检测各组血浆NT-pro BNP水平。对比各组血浆NT-pro BNP水平的差异,分析仫佬族CHF患者与血浆NT-pro BNP水平的相关性;结果观察组血浆NT-pro BNP水平明显高于对照组(P<0.05),而仫佬族组和汉族组血浆NTpro BNP水平无明显差异(P>0.05),心功能分级Ⅱ、Ⅲ、Ⅳ级患者血浆NT-pro BNP水平呈逐渐增高趋势(P<0.05),并与心功能分级呈正相关(r=0.912,P=0.006);结论仫佬族CHF与血浆NT-pro BNP水平密切相关,并与心功能分级呈正相关。故仫佬族地区亦可将血浆NT-pro BNP水平作为慢性心力衰竭的一项常规监测指标。Objective To investigate the Mulao people with chronic heart failure （CHF） and N terminal B type natriuretic peptide precursor（NT-proBNP） and their relationship. Methods 30 cases of either Molao or Han patients with chronic heart failure were selected as observation group, while at the same period, 20 cases of Mulao Nationality of healthy persons as control group. The level of plasma NT-proBNP in each group was detected by UPT-3A. The difference of plasma NT-proBNP level were compared, the correlation analysis of Mulao Nationality CHF patients with plasma NT-proBNP level was conducted. Results We observed that the plasma level of NT- proBNP was significantly higher than the control group （ P 〈 0.05 ）, but there was no significant difference in Mulao group and the Hart group for the levels of plasma NT- proBNP（ P 〈 0.05 ）. For cardiac function grade II, III and IV patients, the NT- proBNP level increased gradually （ P 〈 0.05 ）, and also positively correlated heart function（ r = 0. 912 P = 0. 006 ）. Conclusion Molao CHF is closely related with the level of plasma NT-proBNP, and positively correlated with heart function classification. In the Mulao Ethnicity Area, we can also use plasma NT-proBNP level as a routine monitoring index for chronic heart failure.
