影响EH患者HVA患病率的多因素Logistic回归分析

摘　　要：目的探讨原发性高血压患者高尿酸血症（HUA）的患病率及危险因素。方法选取进行健康体检并符合诊断标准的476例原发性高血压患者作为观察对象。采用尿酸酶一过氧化酶偶联法测定患者的血尿酸水平；分别采取单因素分析和多元Logistic回归分析方法，分析年龄、性别等8个因素与高尿酸血症患病率的关系，确定危险因素。结果高尿酸血症患病率为19．12％。单因素分析发现HUA患病率与性别、年龄、高血糖、高脂血症、肥胖、血肌酐或尿素氮升高、吸烟及饮酒有显著的相关性（P〈0．05）。Logistic分析得到上述因素均为HUA患病的危险因素（P〈0．05），其中血肌酐或尿素氮水平升高危险程度最大（OR：8．484），最低的为高血糖（OR：1．699）。结论HUA发生的危险因素提示积极锻炼、及时治疗高脂血症、控制血糖、保持良好的生活习惯可以降低患病的风险。Objective To explore the incidence of hyperuricemia in essential hypertension patients and its risk factors. Methods 476 cases patients with essential hypertension corresponding to diagnosis standards for health physical examination in hospital was research object. The level of uric acid was detected using urate peroxidase coupling method. Single factor analysis and logistic analysis were used to analyze 8 factors including age and sex relations with incidence of hyperuricemia, in order to identify risk factors. Results The incidence ofhyperuricemia was 19.12%. The single factor analysis showed that 8 factors including sex, age, hyperglycemia, hyperlipidemia, obesity, raised serum creatinine or blood urea nitrogen, smoking and drinking were related factors of incidence of hyperuricemia （ P〈0.05 ） . Logistic regression analysis showed factors mentioned above were the risk factors of incidence of hyperuricemia （ P〈0.05 ） , and the biggest risk factors was raised serum creatinine or blood urea nitrogen （ OR： 8.484 ） , and the least risk factors was hyperglycemia （ OR： 1.699 ） . Conclusion The risk factors of incidence of hyperuricemia shows that positive exercise, timely treatment of hyperlipidemia, controlling of blood sugar level and keeping good living habits can reduce of risk of the diseases.
