基于Logistic回归分析的乙肝后肝硬化中医症状组合规律研究

摘　　要：目的:Logistic回归分析方法寻找出乙肝后肝硬化常见证候特异性的主要及次要症状指标,为该病的中医辨证标准化提供依据。方法:首先应用聚类分析方法对276例乙肝后肝硬化患者的症状、体征分析得出该病的基本中医证候,然后应用Logistic回归分析方法对各证的症状进行分析研究。结果:聚类分析得出5类中医基本证候;Logistic回归分析提示:湿热内蕴证主症:舌红、苔黄腻、脉滑数、脉弦数等,次症:烦躁易怒、渴喜冷饮等;肝肾阴虚证主症:舌红少苔、脉细数、五心烦热等,次症:口燥咽干、花剥苔等;肝郁脾虚证主症:情绪抑郁、脉弦、便溏等,次症:苔薄白、胃脘胀痛等;脾虚湿盛证主症:苔白腻、头身困重、脉濡缓等,次症:脘腹胀满、口腻等;脾肾阳虚证主症:脉沉细弱、舌淡胖边齿痕、畏寒肢冷等,次症:苔白滑、尿少等。结论:Logistic回归分析方法筛选出乙肝后肝硬化基本中医证候的主要及次要症状,为该病中医辨证标准化研究提供了一种新的思路。Objective： To investigate the hepatitis B cirrhosis symptoms rule of combination, provide the ba- sis for the standardization of TCM syndrome differentiation of the disease. Methods： Cluster analysis methods was applied to summarize the hepatitis B cirrhosis basic of TCM syndrome of 276 patients, a comprehensive analysis of hepatitis B cirrhosis symptoms in each card to find out hepatitis B cirrhosis Common Syndromes; Logistic re- gression analysis method was applied to study the specific symptom index. Results： Using cluster analysis, five conunon syndrome types were found. Tip： The main symptoms of damp heat syndrome： red tongue, yellow and greasy fur, slippery pulse number, pulse string number, endoretention of dampheat secondary symptoms： irritabili- ty, thirst hi cold. The main symptoms of liver kidney Yin deficiency syndrome： red tongue, less moss, pulse breakdown, Fanre, liver and secondary symptoms of kidney Yin deficiency symptoms： dry mouth and throat, exfo- liative fur. Main symptoms of liver stagnation and spleen deficiency syndrome： depression, pulse string, loose stool, liver stagnation and spleen deficiency secondary symptoms： thin white fur, stomach pain. Dampness due to spleen deficiency main symptoms： white greasy tongue coating, heavy in the head and body, pulse moisten relief, and dampness due to spleen deficiency symptoms： abdominal fullness, greasy mouth. Spleen kidney Yang deft- ciency main symptoms： thin pulse, pale tongue edge teeth marks, aversion to cold, spleen kidney yang deficiency syndrome secondary： white fur slippery, oliguria. Conclusion： Logistic regression analysis method to screen the main and secondary- symptoms of hepatitis B cirrhosis Basic Chinese medicine, to provide a new way of thinking for the standardization of TCM syndrome differentiation of the disease.
