不同营养支持方案在呼吸重症监护室呼吸肌疲劳患者的应用效果

摘　　要：目的对比分析两种营养支持方案在呼吸重症监护室呼吸肌疲劳患者的不同应用效果。方法选取2016年5月~2017年5月在我院就诊的呼吸重症监护室呼吸肌疲劳患者60例,采用随机数字表法分为观察组(n=30)和对照组(n=30)。观察组患者给予肠内肠外营养支持,对照组患者仅给予肠内营养支持,观察两组患者营养支持前后的营养状况指标(清蛋白、前蛋白、血红蛋白、上臂围(TSF)、上臂肌围(AC)、肱三头肌皮褶厚度(MCAM))、免疫功能指标及肺功能指标。结果观察组患者清蛋白、前蛋白、血红蛋白、TSF、AC、MCAM均显著优于对照组(P<0.05);观察组患者FEV1%、FEV1/FVC、MIP、PaCO_2、PaO_2、辅助呼吸肌评分显著优于对照组,差异有统计学意义(P<0.05)。结论肠内肠外营养支持能够有效改善呼吸重症监护室呼吸肌疲劳患者的营养状况,改善部分免疫功能,以提高患者的呼吸肌能力,提高肺功能,具有临床推广使用价值。Objective To compare and analyze the different application effects of two nutrition support schemes in respiratory intensive care patients with respiratory muscle fatigue.Methods 60 cases of respiratory muscle fatigue were selected from May 2016 to May 2017 in our hospital.Adopting digital random method,were divided into observation group （n=30） and control group （n=30）,the observation group patients give enteral parenteral nutrition support,control group patients give enteral nutrition support,only watch two groups of patients before and after nutritional support of nutrition indicators,（former protein albumin,hem0globin,upper arm circumference （TSF） and arm muscle circumferenee（AC）, triceps skinfold （MCAM））,immune function index and pulmonary function index.Results The thickness of albumin,preprotein, hemoglobin,upper arm circumference,upper arm circumference,and triceps of the triceps were significantly better than the control group,the dfference was statistically significant （P〈0.05）.In the observation group,FEVl%,FEV/FVC, MIP,PaCO2,PaO2,and auxiliary respiratory muscle score were significantly better than the control group,the difference was statistically significant （P〈0.05）.Conclusion Enteral parenteral nutrition support can effectively improve the respi- ratory intensive care unit well fatigue nutritional status of patients,improve immune function, in order to improve the well ability of patients, improve pulmonary function, clinical use value.
