皮下置引流管预防肥胖患者腹部手术切口感染的研究

摘　　要：目的探讨皮下放置负压引流管对预防肥胖患者急诊剖腹探查术后切口感染的作用。方法选取具有可比性的急诊行剖腹探查术的肥胖患者240例,切口类别为Ⅱ类及Ⅲ类切口,随机分为实验组及对照组各120例,缝合切口时实验组皮下放置负压引流管,对照组常规缝合,观察两组患者切口甲级愈合情况、切口感染率、二期缝合情况及住院时间。结果切口甲级愈合实验组为110例,对照组为90例,实验组明显高于对照组(P<0.01);切口感染实验组3例,对照组11例,实验组明显低于对照组(P<0.05);二期缝合实验组2例,对照组9例,实验组明显低于对照组(P<0.05);住院时间实验组为(7.72±3.144)d,对照组为(12.28±4.512)d,实验组明显低于对照组(P<0.01)。结论肥胖患者急诊行剖腹探查术Ⅱ类及Ⅲ类切口缝合切口时皮下放置负压引流管能提高切口甲级愈合率,降低切口感染率,降低二期缝合率及缩短患者住院时间,值得临床进一步推广应用。Objective To investigate the effect of subcutaneous placement of negative pressure drainage tube on prevention of incision infection after exploratory laparotomy in obese patients.Methods Selecting 240 obese patients in a comparable emergency laparotomy,with incision type both class Ⅱand class Ⅲ incision.And they were randomly divided into experimental group and control group,with 120 cases in each group.Incision suture in experimental group had subcutaneous negative pressure drainage tube.Routine suture was used in control group.Wound healing,incision infection,suture and hospitalization of two groups were compared.Results There were 110 cases in wound healing in experimental group,and 90 cases in the control group.The experimental group was significantly higher than the control group（P〈0.01）.There were 3 cases of wound infection in the experimental group and 11 cases in the control group.The experimental group was significantly lower than the control group（P〈0.05）; There were 2 cases in the experimental group and 9 cases in the control group having the second period suture.The experimental group was significantly lower than that of the control group（P〈0.05）.Hospitalization time of the experimental group was（7.72 ± 3.144） days,the control group was（12.28 ± 4.512） days.The experimental group was significantly lower than the control group（P〈0.01）.Conclusion Suture incision subcutaneous negative pressure drainage tube in obese patients with emergency laparotomy Ⅱ class and Ⅲ class incision can improve the healing rate of the second period incisions,decrease the infection rate of the second period incision and suture,and shorten the hospitalization time.So it is worthy of further clinical application.
