摘　　要：目的对比分析艾滋病病毒(HIV)抗原抗体联检电化学发光法(ECL)反应性结果与蛋白印迹法(WB)确证实验检测结果,找出ECL诊断HIV阳性病人的阈值,探讨HIV检测新策略。方法收集郑州大学第一附属医院2012年4月至2016年2月646例病人复筛ECL及HIV确证试验结果,对23例WB不确定结果进行随访确诊,回顾性分析两组检验资料;采用受试者工作特征曲线(ROC)评估ECL检测HIV抗体的能力。结果 ECL检测方法的总阳性率为59.44%(384/646),ECL诊断HIV抗体的ROC曲线下面积为0.990,cutoff值32.17,灵敏度97.10%,特异度96.20%;以cutoff值为界值,COI≥32.17的病人中97.14%为真阳性,COI≥200时,确证实验阳性率为100%,COI<32.17的病人中96.23%为假阳性。结论 ECL检测值COI越大,WB的阳性率越高,该结果可为采用酶联免疫试验检测HIV抗体的检测策略提供重要的参考依据。Objective To analyze the correlation between the results of human immunodeficiency virus（HIV） antigen antibody combination assay of electro-chemiluminescence（ECL） and western blot（WB） method, and explore the possibility of the ECL instead of western blot for HIV confirmation test. Methods A total of 646 patients were selected from April 2012 to February 2016 in the first affiliated hospital of Zhengzhou university and the results of the fourth generation of electrochemiluminescence for their HIV confirmatory test were got. 23 cases of WB uncertainty were followed up, and the data were analyzed retrospectively. Receiver operating curve（ROC） was used to evaluate the effect of the fourth generation ECL for HIV detection. Results The positive rate of ECL detection was 59.44%. The area under the curve（AUC） was 0.990. The cutoff value was 32.17, with the sensitivity of 97.10%, and specificity of 96.20%. In the case of the cutoff value as the limit value, 97.14% cases with COI≥32.17 were proved to be true-positive HIV infection; with COI≥200, 100% of cases were the positive as confirmatory test, and with COI〈32.17, 96.23% cases were falsely positive. Conclusion The greater the value of ECL, the higher the positive rate of WB is, which can provide an important basis for HIV detection strategy by using enzyme linked immunosorbent assay to diagnose HIV antibody.
