右美托咪定在老年髋关节置换术中镇静及遗忘效果的临床观察

摘　　要：目的观察及分析右美托咪定在老年患者髋关节置换术中镇静及遗忘效果。方法选取120例髋关节置换手术患者为研究对象,随机分为右美托咪定组和对照组各60例,入室后右美托咪定组患者予右美托咪定持续静脉泵注,对照组患者予生理盐水同等速度持续输注,手术结束后,比较两组患者镇静及遗忘效果。结果 T_1、T_2、T_3、T_4时,右美托咪定组患者SBP、DBP、MAP、HR值明显低于对照组患者,差异有统计学意义(P<0.05),且各时段右美托咪定组患者SBP、DBP、MAP、HR值均明显低于T_0时段,右美托咪定组患者镇静OAA/S评分和BIS值明显低于对照组患者,差异有统计学意义(P<0.05)。右美托咪定组患者遗忘率100%,显著高于对照组患者遗忘率26.67%,差异有统计学意义(Z=-8.475,P<0.001)。结论右美托咪定应用于老年患者髋关节置换术,术中镇静及遗忘效果确切。Objective To observe and analyze the effect of dexmedetomidine on sedation and amnesia in elderly patients undergoing total hip arthroplasty. Methods 120 cases of patients with hip replacement surgery were randomly divided into two groups,60 cases in each group,after entering the dexmedetomidine group was given dexmedetomidine continuous intravenous infusion,and the control group was treated with saline of equal speed continuous infusion,after the surgery,the sedative and forgetting effect contrast.Results T_1,T_2,T_3,T_4,SBP,DBP,MAP,HR values of dexmedetomidine group were significantly lower than the control group,the difference was statistically significant（ P〈0. 05）,and each time the patients in dexmedetomidine group SBP,DBP,MAP,HR values were significantly lower than the T0 period,right dexmedetomidine group OAA/S sedation score and BIS value was significantly lower than the control group,the difference was statistically significant（ P0.05）. The amnesia rate of the dexmedetomidine group was 100%,which was significantly higher than that of the control group,and the rate of forgetting was 26. 67%. The difference was statistically significant（ Z=-8.475,P0.001）.Conclusions Dexmedetomidine should be used in elderly patients with hip replacement surgery,sedation and amnesia effect is accurate,worthy of clinical use and promotion.
