腹腔镜手术治疗小儿腹股沟斜疝120例

摘　　要：目的探讨腹腔镜下疝囊高位结扎术联合覆盖脐内侧襞术式用于治疗小儿腹股沟斜疝的临床应用价值及经验总结。方法回顾性分析2016年7月-2017年7月天津市儿童医院腹腔镜疝囊高位结扎术联合覆盖脐内侧襞治疗腹股沟斜疝患儿120例的临床资料,包括术中出血量、手术时间、住院时间,以及术后并发症和复发情况。结果 120例患儿均顺利完成手术,无中转开放式手术,术中发现对侧隐匿疝17例,手术时间平均21.5min(10~50min),术中出血量平均1.1ml(0.5~1.5ml),平均出院时间为3.5d(2~4d),术后无手术并发症,术后随访2~12月无复发病例。结论腹腔镜下疝囊高位结扎术联合覆盖脐内侧襞治疗小儿腹股沟斜疝创伤小、恢复快、并发症少、复发率低,且安全可靠,值得临床推广应用。Objective To explore the clinical application value and experience of laparoscopic hernial cys tic high ligation combined with umbilical medial pleura in the treatment of inguinal oblique hernia in children. Methods The clinical data of 120 children with inguinal oblique hernia treated with laparoscopic hernial cystic high ligation combined with umbilical medial pleura in Tianjin Children＇s Hospital from July2016 to July 2017 were retrospective analysed,including intraoperative bleeding volume,operation time,hospital stay,and postoperative complications and recurrence. Results All the 120 patients were successfully completed with the operation,without open conversion surgery. 17 cases of contralateral concealed hernia were found during the operation. The average operative time was 21.5 min（10-50 min）.The average intraoperative blood loss was 1.1 ml（0.5-1.5 ml）. The average hospital discharge time was 3. 5 d（2-4 d）. There were no postoperative complications,and there was no recurrence in 2-12 months after the operation. Conclusion The treatment of laparoscopic hernial cystic high ligation combined with umbilical medial pleura for indirect inguinal hernia in children was with less damage,quick recovery,low complication rate and low recurrence rate,and it is safe and reliable and is worth promoting.
