老年糖尿病患者社区精细化管理的效果分析

摘　　要：目的 评价对老年糖尿病患者实施社区精细化管理后的效果.方法 将104例符合条件的老年2型糖尿病患者按随机数字表法分为观察组和对照组各52例.观察组纳入签约服务,采用社区精细化管理方式进行干预,对照组采用社区常规管理.结果 干预6个月后,两组患者的自我管理能力及有关代谢指标较干预前均有改善（均P〈0.05）,但观察组效果更明显,差异有统计学意义（P〈0.05）.结论 社区精细化管理能够有效提高老年糖尿病患者的自我管理能力,改善相关代谢指标.Objective To observe the effect of community fine management in the elderly patients with diabe-tes.Methods A total of 104 patients with elderly type 2 diabetic were randomly divided into observation group and con-trol group, with 52 cases in each group. The observation group was included in the service contract, and was given the fine management of community intervention, while the control group was given the routine management of communi-ty.Results After 6 months＆#39;intervention, the self-management ability and related metabolic indexes of the two groups were improved（P〈0.05）, but the effect of the observation group was more obvious, and the difference was statistical-ly significant （P〈0.05）.Conclusions The meticulous management can improve the self-management ability and the related metabolic indexes of the elderly patients with diabetes.
