顺应宫缩自然娩肩法预防肩难产及新生儿产伤的效果探讨

摘　　要：目的探讨顺应宫缩自然娩肩法预防肩难产及新生儿产伤的效果。方法选取我院2016年6月~2017年6月收治的自然分娩产妇200例,按照随机数字表法分为研究组和对照组,对照组应用传统娩肩方法进行分娩,研究组应用顺应宫缩自然娩肩法进行分娩,比较分析不同组别产妇分娩结局和新生儿分娩结局。结果不同组别产妇分娩结局情况对比,研究组产妇应用顺应宫缩自然娩肩法分娩后,产妇分娩结局中产后出血、会阴伤口延伸、产褥感染及阴道会阴裂伤发生率均更低;同时产妇肩难产发生率也更低;新生儿产伤情况中锁骨骨折、颅内出血发生率均更低;同时研究组新生儿Apgar评分总窒息发生率和行为异常发生率均更低,上述指标数据组间比较差异均有统计学意义(P<0.05)。研究组产妇分娩结局及新生儿结局均较好。结论顺应宫缩自然娩肩法能够有效预防肩难产情况的发生,利于降低新生儿产伤,具有较高的临床应用价值。Objective To explore the effect of adaptation shoulder contractions continuous delivery to prevent shoulder dystocia and neonatal injury. Methods 200 cases of natural childbirth women were selected,and according to the random number table method were divided into study group and control group in hospital from June 2016 to June 2017.The control group was given the traditional delivery method.The study group was given the adaptation shoulder contractions continuous delivery.The maternal childbirth outcome and neonatal delivery outcome were compared between the 2 groups. Results Compared with the control group, postpartum hemorrhage,perineal wound extension,puerperal infection and vaginal laceration incidence were obviously decreased in the study group,while the maternal shoulder dystocia rate was lower,neonatal injury cases of clavicle fracture,incidence of intracranial hemorrhage were lower in the study group ,and total Apgar score of neonates asphyxia and abnormal behavior the incidence rate were lower in the study group,the differences were statistically significant（P 〈 0.05）.The birth outcomes and neonatal outcomes of the study group were better. Conclusion The adaptation shoulder contractions continuous delivery can effectively prevent the occurrence of shoulder dystocia,and is conducive to reducing neonatal birth injury,and has high clinical value.
