手法扩张阴道辅以石蜡油润滑降低产妇会阴侧切率及减轻会阴裂伤程度的探讨

摘　　要：目的探讨分娩中徒手扩张阴道辅以石蜡油润滑降低产妇会阴侧切率及减轻会阴裂伤程度的效果。方法随机选取我院2017年1~9月间正常阴道分娩的初、经产妇200例分成对照组与观察组(每组包括25例初产妇,75例经产妇)。对照组采取常规分娩方式进行接生,不采取手法扩张阴道辅以石蜡油润滑。观察组采取常规分娩方式进行接生,但在胎头将拔露时采取手法扩张阴道辅以石蜡油润滑。观察两组患者会阴侧切率及裂伤程度。结果对照组会阴无裂伤20例,占20%,Ⅰ度裂伤29例,占29%,Ⅱ度裂伤31例,占31%,会阴侧切20例,占20%;观察组会阴无裂伤38例,占38%,Ⅰ度裂伤41例,占41%,Ⅱ度裂伤14例,占14%,会阴侧切7例,占7%。相较于对照组,观察组中的产妇会阴侧切率以及裂伤程度更低,两组比较差异有统计学意义(P<0.05)。结论对分娩产妇采取手法扩张阴道辅以石蜡油润滑,能够有效减少初、经产妇会阴侧切率及减轻会阴裂伤程度,值得临床积极推广。Objective To investigate the effect of manual dilatation of vagina and paraffin oil lubrication on the reduction of episiotomy rate and the perineal laceration degree. Methods 200 cases of normal vaginal delivery in our hospital from January 2017 to September 2017 were randomly divided into the control group and the observation group （each group consisted of 25 cases of primipara and 75 cases of parturients）.The control group was treated by routine delivery,and not taken manual dilation of the vagina,supplemented by paraffin oil lubrication.The observation group was delivered by routine delivery method,but the technique was performed by expanding the vagina and lubricating with paraffin oil when the fetal head was pulled out.The rate of episiotomy and the degree of laceration were observed. Results The control group without perineal laceration accounted 20 cases,accounted for 20%,and I degree laceration 29 cases,accounted for 29%,and H degree laceration accounted 31 cases,accounted for 31%, and perineal resection accounted 20 cases,accounted for 20%.The observation group without perineal laceration accounted 38 cases,accounted for 38%,and I degree laceration accounted 41 eases,accounted for 41%, and II degree laceration accounted 14 cases,accounted for 14%,and perineal resection accounted 7 cases,accounted for 7%.Compared with the control group,the rate of episiotomy and the laceration degree of observation group were lower.The difference was statistically significant （P 〈 0.05）. Conclusion Taking vaginal dilation with paraffin oil lubrication for the parturient women, can effectively reduce the early pregnancy, the rate of lateral episiotomy and alleviate perineal laceration, worthy of clinical promotion actively.
