无创通气联合糖皮质激素应用在急性间质性肺炎中的治疗效果研究

摘　　要：目的探讨无创通气联合糖皮质激素应用在急性间质性肺炎中的应用效果。方法选取2012年2月～2016年12月我科收治的急性间质性肺炎患者共计62例为研究对象,根据不同的治疗方案分为两组,观察组给予在常规对症治疗的基础上,采取无创通气联合糖皮质激素治疗,对照组予以糖皮质激素治疗,治疗后统计患者体温明显下降、肺部啰音好转以及CT肺部感染好转时间;计算患者治疗有效率;记录两组患者炎症指标,C反应蛋白,血沉及中性粒细胞百分比变化情况以及并发症的发生情况,并进行统计学分析。结果观察组在体温明显下降时间为(3.21±0.54)d、肺部啰音明显好转时间为(6.21±2.32)d、肺部CT明显好转时间为(13.58±0.99)d,治疗有效率达84.38%、炎症指标下降程度,并发症的发生率方面结果均优于对照组,且差异具有统计学意义(P<0.05)。结论无创通气联合糖皮质激素治疗急性间质性肺炎效果显著,值得临床上优先考虑应用。Objective To explore the effect of noninvasive ventilation combined with glucoeorticoids in the treatment of acute interstitial pneumonia. Methods 62 patients with acute interstitial pneumonia in our hospital from February 2012 to December 2016 were selected in the study.And they were divided into two groups according to different treatment options.The observation group was treated with noninvasive ventilation combined with gluencortieoid treatment on the basis of conventional symptomatic treatment,and the control group was treated with glucocorticoid.After treatment,the time of body temperature decline significantly,improvement of pulmonary roles and improvement of CT pulmonary infection were recorded.The effective rate of treatment was calculated;The inflammatory indexes,C reactive protein,ESR and percentage of nentrophils,and the incidence of complications in two groups were recorded.And statistical analysis was carried out. Results The time of body temperature decline significantly,improvement of pulmonary rales and improvement of CT pulmonary infection were （ 3.21 ± 0.54 ）, （ 6.21 ± 2.32 ）, （ 13.58 ± 0.99 ）d in the observation group.The effective rate of the observation group was 84.38%.The decrease degree of inflammatory indexes and the im;idence of eomplicatious were better than those of the control group,and the difference was statistically significant （P 〈 0.05）.Conclusion The effect of noninvasive ventilation combined with glucocortieold in the treatment of acute interstitial pneumonia is significant,which is worthy of clinical application.
