摘  要:目的:探讨0.75%罗哌卡因不同速度注入蛛网膜下腔对子宫全切除术中牵拉反应发生的影响。方法:收治子宫肌瘤择期行子宫全切除术患者60例,随机分为3组,各20例。均使用0.75%罗哌卡因行腰麻联合硬膜外麻醉,腰麻注药速度分别为0.1 mL/s(LR组),0.2 mL/s(MR组),0.3 mL/s(HR组)。结果:3组患者术中牵拉宫颈时恶心/呕吐、鼓肠、肌紧张等的发生率,平均动脉血压、心率,需辅助静脉麻醉药物及血管活性药物例数差异有统计学意义(P<0.05);3组患者运动阻滞维持时间及阻滞效果、手术时间比较,差异有统计学意义(P<0.05)。结论:0.75%罗哌卡因不同速度注入蛛网膜下腔在子宫全切除术中发生牵拉反应具有差异,0.1 mL/s注药速度者牵拉反应发生率最低。Objective:To investigate the effect of 0.75% ropivacaine injected into the subarachnoid space at different speeds on the occurrence of traction reaction during total hysterectomy.Methods:60 patients with hysteromyoma undergoing elective total hysterectomy were selected.They were randomly divided into the three groups,with 20 cases in each group.All of the patients were used 0.75% ropivacaine for spinal anesthesia combined with epidural anesthesia.The rate of spinal anesthesia was 0.1 mL/s(the LR group),0.2 mL/s(the MR group),0.3 mL/s(the HR group).Results:Cervical traction,nausea/vomiting,drum,intestinal muscle tension and other incidence in the three groups were statistically significant,and also comparison of mean arterial pressure,heart rate,and required intravenous anesthetic and vasoactive drugs were statistically significant(P〈0.05).The differences in the motor block duration,block effect and the operation time was statistically significant(P〈0.05).Conclusion:0.75% ropivacaine injected into the subarachnoid space at different speeds has a difference in the total traction response during total hysterectomy,and the incidence of traction reaction is the lowest at 0.1 mL/s injection speed.
