脑卒中后认知功能障碍患者动态血压特点

摘　　要：目的回顾性分析脑卒中后认知功能障碍(PSCI)合并高血压患者急性期动态血压特点。方法选取2015年7月至2016年2月医院收治的216例急性缺血性脑卒中合并高血压患者,按简易精神状态评价量表(MMSE)评分结果分为PSCI组和非认知功能障碍组,比较两组24 h血压参数,认知功能障碍组治疗前后相关参数及MMSE评分变化。结果认知功能障碍组24 h平均收缩压(24 h m SBP)、24 h平均舒张压(24 h m DBP)、24 h收缩压变异性(24 h SBPV)、24 h舒张压变异性(24 h DBPV)、非(反)杓形比例均高于非认知功能障碍组,差异有统计学意义(P<0.05);认知功能障碍组经多奈哌齐治疗2周后,24 h收缩压变异性、24 h舒张压变异性、非(反)杓形比例均改善,差异有统计学意义(P<0.05),但MMSE分数比较,差异无统计学意义(P>0.05)。结论 PSCI患者血压平均水平可能更高,血压变异性可能更大,早期干预可能获益。Objective To analyze the ambulatory blood pressure of patients with post-stroke cognitive impairment and hypertensionin the first stroke acute phaseretrospectively. Methods From July 2015 to February 2016,216 patients who wereischemic stroke accompanied with hypertension were selected as the research objects. They were divided into cognitive impairmentgroup and normal group in accordance with the score results of MMSE. The 24 h blood pressure parameters of two groups were compared,and the related parameters and MMSE score were observed before and after 2 weeks in cognitive dysfunction group. Results The 24 h mSBP,24 h mDBP,24 h SBPV,24 h DBPV,non dipper and anti dipper ratios of the cognitive impairmentgroup were higher than those of the normal group,the differences were statistical y significant（ P 0. 05）. The 24 h SBPV,24 h DBPV,non dipper and anti dipper ratios improved after 2 weeks of donepezil treatment in the cognitive impairmentgroup,the differences were statistically significant（ P 0. 05）,but there was no significant change in MMSE scores（ P 0. 05）. Conclusions The average level of blood pressure and the blood pressure variability may be higher in patients with post-stroke cognitive impairment. If we intervene early enough,thepatientmaybenefit.
