蒙脱石散联合多维元素片局部治疗小儿复发性口腔溃疡的临床研究

摘　　要：目的观察蒙脱石散联合多维元素片局部治疗小儿口腔溃疡的疗效及对血清白细胞介素-2(IL-2)、肿瘤坏死因子(TNF-α)的影响。方法 120例反复发生口腔溃疡患儿,按数字表法随机分为对照组(59例)和观察组(61例)。对照组予双料喉风散局部喷敷治疗;观察组采用蒙脱石散加多维元素片研磨成粉涂抹患处。均每日3~4次。观察两组临床疗效、治愈时间、不良反应发生率、细胞因子水平及复发率。结果观察组总有效率为95.1%(58/61),显著高于对照组(χ~2=5.214,P<0.05)。观察组治愈时间为(3.56±0.74)d,复发率为4.3%,与对照组比较差异均有统计学意义(P<0.05)。治疗后观察组IL-2显著高于对照组,TNF-α显著低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论蒙脱石散加复合维生素片研磨成粉末局部治疗小儿复发性口腔溃疡疗效显著,能明显缩短治愈时间,改善炎性细胞因子水平,降低复发率且安全性好。OBJECTIVE To observe the clinical efficacy of montmorillonite powder and multivitamin with multimineral tablets for local use in the treatment of children with oral ulcer,and their effects on IL-2 and TNF-α. METHODS A total of 120 children with recurrent oral ulcer were selected and divided into treatment group（ n = 61） and control group（ n = 59） according to random number table. Control group was given superior sore throat powder for local use,3-4 times per day; treatment group was given with mixed power of montmorillonite powder and multivitamin with multimineral tablets,3-4 times per day. Clinical efficacy,the time of ulcer disappearance,the incident of ADR,recurrence rate of ulcer and cytokines degree were compared between the two groups. RESULTS Total response rate of treatment group was 95.1%（ 58/61）,which was significantly higher than control group（ χ~2= 5.214,P〈0.05）.The time of ulcer disappearance was（ 3. 56 ± 0. 74） d,and recurrence rate of ulcer was 4. 3%,with statistical significance（ P〈0.05） in comparison to control group. After treatment,IL-2 degree was significantly higher than control group,and TNF-α degree was significantly lower than control group,with statistical significance（ P〈0.05）. There was no statistical significance in the incident of ADR between two groups（ P〉0.05）. CONCLUSION Mixed power of montmorillonite powder combined with multidimensional element tablets for local use have obvious effect on oral ulcer and with good safety in children.
