肠道病毒不同亚型与手足口病并发热性惊厥相关性的病例对照研究

摘　　要：目的研究EV71和非EV71肠道病毒感染所致手足口病并发热性惊厥的危险性是否存在差异,以及EV71感染致神经系统受累后惊厥发生的危险性是否增加。方法选取2015年我科住院的手足口病患儿,有惊厥者为病例组,无惊厥者为对照组,以不同肠道病毒亚型为暴露因素,进行回顾性病例对照研究;同时EV71感染者按有无神经系统受累、有无惊厥进行分组对照分析。结果共有1 107例手足口病患儿纳入研究,EV71感染445例,惊厥7例;非EV71感染662例,惊厥62例(χ~2=24.79,P<0.01,OR=6.46),非EV71感染手足口病并发热性惊厥的危险性是EV71感染者的6.46倍。EV71感染神经系统受累215例,惊厥2例;无神经系统受累者230例,惊厥5例,两组病例出现惊厥的比例差异没有统计学意义(χ2=0.45,P>0.05)。结论不同肠道病毒分型与手足口病并发热性惊厥的危险性有关联,非EV71型肠道病毒感染所致手足口病并发热性惊厥的危险性显著高于EV71感染,神经系统受累并不增加EV71感染者惊厥的发生率。Objective To investigate whether the risk of hand foot mouth disease（ HFMD） complicated with febrile seizures caused by EV71 and non-EV71 enterovirus infection is different and whether the risk of seizures increased after EV71 infection is involved in nervous system. Methods The pediatric patients with HFMD in our department in 2015 were divided into case group and control group according whether being complicated with seizures. A retrospective case-control study was performed with different intestinal virus subtype as exposure factors. EV71 infection was grouped and analyzed according to whether the nervous system was involved and the seizure occurred. Results A total of 1 107 cases of HFMD were enrolled in the study,in which,there were 445 cases of EV71 infection with 7 cases of convulsion and 662 cases of non-EV71 infection with 62 cases of seizures（ χ2= 24. 79,P〈0. 01,OR = 6. 46）. The risk for HFMD and febrile seizures of patients with non-EV71 infection were 6. 46 times of those with EV71 infection.There were 215 cases of nervous system involvement,including 2 cases of seizures,and 230 cases of non-nervous system involvement,including 5 cases of seizures,no significant difference was found in the number of seizures between them（ χ2= 0. 45,P〈0. 05）. Conclusion There is correlation between different enterovirus type and HFMD complicated with febrile seizures,the risk for febrile seizures of HFMD induced by enterovirus infection of non-EV71 was significantly higher than that of EV71 infection. Nervous system involvement does not increase the incidence of convulsion induced by EV71 infection.
