独活寄生汤结合单侧穿刺椎体后凸成形术治疗腰椎骨质疏松性压缩骨折疗效分析

摘　　要：目的:探讨术前口服独活寄生汤结合单侧穿刺椎体后凸成形术(PKP)治疗腰椎骨质疏松性压缩骨折(LOVCF)临床疗效。方法:回顾性分析2013年5月—2013年12月中药口服结合单侧穿刺PKP治疗48例OVCF患者的临床资料。用疼痛视觉模拟评分(VAS),骨折椎前缘、中部高度,Cobb'S角评价经横突一椎弓根单侧穿刺PKP的临床疗效。结果:32例患者手术顺利,伤椎内骨水泥分布均匀,术后伤口愈合良好,未出现神经根损伤,其中两例出现骨水泥椎体前方外溢但未出现严重并发症。45例患者获得随访,随访时间10~24个月,平均18个月。与术前相比,术后VAS评分明显好转,后凸角度明显变小,椎体前缘、中部高度有明显恢复(P<0.05)。结论:口服中药结合单侧穿刺PKP是治疗OVCF的一种有效方法。Objective ： To explor the curative effect of Duhuo Jisheng Decoction combined with unilateral puncture kyphoplasty （ PKP ） on treating osteoporotic compression fracture of lumbar vertebra （ LOVCF ）. Methods ： The clinical materials of 48 patients with OVCF, treated by PKP from unilateral transverse process pedicle approach, were retrospectively reviewed from May 2012 to December 2012. The clinical efficacy was evaluated by visual analogue scale （ VAS ）, anterior mudium height of vertebrae body and Cohb＇s angle in preoperation and postopereation. Results ： PKP was successful in 45 patients without severe complications, such as nerve root injury, cement leakage into spinal canal. Cement was well distributed in vertebrate. 45 patients were followed up for 10-24 months （ average 18 months ） . There were significant defferences in VAS, anterior/mudiem herght of wertebrac body and Cobb＇s angle between preaperation group and postoperation group, P〈0.05. Conclusion ： Oral Chinese medicine combined with PKP surgery is an effective way to treat LOVCF.
