摘  要:目的:探讨补肾固精续骨方对骨折延迟愈合患者促愈作用疗效机制。方法:选取80例中老年四肢骨折延迟愈合患者为研究对象,采用随机数字表法分为对照组及治疗组各40例。两组均予体外冲击波治疗,治疗组再予补肾固精续骨方中药汤剂口服。观察两组治疗前后血清骨代谢[骨形成蛋白(BMP2)、骨钙素(BGP)、胰岛素生长因子-Ⅰ(IGF-Ⅰ)]水平、骨痂形成的变化,评估临床疗效。结果:两组治疗后血清BMP2、BGP、IGF-Ⅰ水平均低于同组治疗前,组内差异有统计学意义(P<0.01);治疗后治疗组血清BMP2、BGP、IGF-Ⅰ水平均低于同期对照组,组间差异有统计学意义(P<0.01)。两组治疗结束后骨痂形成评分呈逐渐增加趋势;治疗组在治疗结束时、治疗结束后4周、治疗结束后12周骨痂评分均高于对照组,组间差异有统计学意义(P<0.01)。治疗组愈显率80.0%,对照组52.5%,组间差异有统计学意义(P<0.01)。结论:补肾固精续骨方能促进骨折延迟愈合患者骨痂形成及骨折愈合,其机制可能与改善血清骨代谢水平相关。Objective: To investigate the effect of Bushen Gujing Recipe (补肾固精续骨方, BSGJR) on promoting healing in patients with delayed union of fracture. Methods: 80 cases of middle and old aged patients with delayed union of fracture of limbs were selected as the object of study. They were randomly divided into control group (n=40 cases) and study group (n=40 cases). The two groups were treated with extracorporeal shock wave, and the study group was given BSGJR. The changes of serum bone metabolism [bone morphogenetic protein (BMP2), osteocalcin (BGP), insulin growth factor 1 (IGF- 1)] level and formation of callus were observed before and after treatment in two groups, and the clinical efficacy was evaluated. Results: The serum BMP2, BGP, IGF- lof the two groups after treatment were significantly lower than that of the same group before treatment, the differences within groups (P〈0.01); the study group after treatment serum BMP2, BGP, IGF- I levels were lower than the control group (P〈0.01). The scores of callus formation increased gradually after two treatments. The scores of callus in the study group were higher than those in the control group at the end of the treatment, 4 weeks after the treatment and 12 weeks after the end of treatment (P〈0.01). The effective rate of the study group was 80%, 52.5% in the control group, the difference was significant (P〈0.01). Conclusion: BSGJR can promote callus formation and fracture healing in patients with delayed fracture healing, and its mechanism may be related to the improvement of serum bone metabolism level.
