摘 要:目的探讨芳香烃受体(AhR)在SLE患者外周血淋巴细胞中的变化及其与SLE疾病的关系,评估AhR在SLE疾病诊断中的价值。方法收集SLE患者20例和健康志愿者22例,采用流式细胞术及实时荧光定量PCR(q RT-PCR)方法对AhR的表达情况进行检测,并与SLEDAI-2K评分、ESR、补体C3和C4及CRP做相关性分析及二元Logistic回归分析。结果 SLE患者外周血AhR阳性淋巴细胞占淋巴细胞比值(10.76±4.52)%显著高于健康对照组(0.45±0.20)%;SLE患者外周血淋巴细胞中AhR mRNA表达水平(0.21±0.14)显著高于健康对照组(0.05±0.01),差异均有统计学意义(P<0.01)。外周血AhR阳性淋巴细胞占淋巴细胞比值与SLE患者血清补体C3(r=-0.53,P<0.05)、C4(r=-0.63,P<0.05)呈负相关,与ESR呈正相关(r=0.55,P<0.05),而与CRP(r=-0.36,P>0.05)和SLEDAI-2K评分(r=-0.06,P>0.05)无相关性。二元Logistic回归分析结果显示,AhR阳性淋巴细胞占淋巴细胞比值[EXP(B)=3.337,P=0.001]、ESR[EXP(B)=1.354,P=0.018]是SLE疾病的独立危险因素,且AhR阳性淋巴细胞占淋巴细胞比值相较于ESR统计学差异更显著。结论外周血淋巴细胞中的AhR可用于评估SLE疾病的活动性并可用于SLE疾病的诊断,淋巴细胞中高表达的AhR可能是促进SLE发病的重要因素。This study was designed to evaluate the value of aryl hydrocarbon receptor(AhR) in the diagnosisof SLE by exploring the changes of AhR in peripheral blood lymphocytes and its association with disease activity inpatients with SLE.Flow cytometry and Real-time quantitative PCR(q RT-PCR) were applied to detect AhRexpression in 20 SLE cases and 22 healthy controls.And then,the correlation of AhR expression level withSLEDAI-2 K,ESR,C3,C4 and CRP were assessed.Data showed that the AhR positive cells in lymphocytes ofperipheral blood were significantly increased in SLE patients(10.76±4.52)%,as compared with healthy controls(0.45±0.20)%(P〈0.01); AhR mRNA level in SLE patients(0.21±0.14) was higher than that in healthy controls(0.05±0.01)(P〈0.01).AhR expression level was negatively correlated with C3(r=-0.53,P〈0.05),C4(r=-0.63,P〈0.05),while positively correlated with ESR(r= 0.55,P〈0.05),but showed no correlation with CRP or SLEDAI-2 K.Binary Logistic regression analysis showed that SLE was independently related to AhR [EXP(B)=3.337,P=0.001]and ESR [EXP(B) =1.354,P=0.018],and the former has more significant statistical difference than ESR.Inconclusion,AhR in peripheral blood lymphocytes can be used to assess the activity of SLE and for the diagnosis ofSLE.High expression level of AhR in lymphocytes maybe an important factor in SLE development.
