基于德尔菲法形成慢性肾脏病风邪证候要素判定专家共识的研究

摘　　要：目的基于德尔菲法形成《慢性肾脏病风邪证候要素判定专家共识》。方法基于德尔菲法,结合前期的文献研究结果,通过2轮专家咨询,从满分率、均数、变异系数以及协调系数等多个角度,对专家咨询结果进行评价,并结合临床经验对条目进行修改,形成慢性肾脏病风邪证候要素判定的专家共识。结果共选取了20名国内著名专家进行问卷调查,专家权威程度得分为17.9,第一轮、第二轮的问卷回收率分别达到95.24%、75.00%。第2轮整体协调系数为0.348,根据各条目的均值以及变异系数,结合临床经验,对条目进行适当删减后,最终确定10个条目:急性发作时伴有外感症状(如咽痛)或因外感诱发而反复发作、尿中大量泡沫、面部浮肿、腰膝酸痛、关节疼痛、头晕耳鸣、伴有皮疹、皮肤瘙痒、进食海鲜等发物可导致病情加重、病情变化快。结论应用德尔菲法形成了《慢性肾脏病风邪证候要素判定专家共识》,该量表代表了大多数专家的观点,权威程度高,协调性较好。Objective To develop the ＂Wind Pathogen Syndrome Element Judging Expert Consensus in Chronic Kidney Diseases＂based on Delphi method. Methods Based on Delphi method,combining with the results of previous literature research,after two rounds of expert consultation,evaluated the results of expert advice from multiple aspects,including full mark rate,mean scores,variation coefficient and the coordination coefficient,then modified the items combined with clinical experience,and finally developed Wind Pathogen Syndrome Element Judging Expert Consensus in Chronic Kidney Diseases. Results Selected 20 domestic famous experts in total for a questionnaire survey. The expert authority score was 17. 9. The recovery rate of the questionnaire in the first round and the second round were 95. 24% and 75. 00% respectively. The overall coordination coefficient of the second round was 0. 348.According to the mean scores and variation coefficient of each item,combined with clinical experience,after appropriate deletion,determined the following 10 items： exogenous symptoms（ such as sore throat） at acute stage or repeated attacks induced by external infection,a large number of bubbles in the urine,facial swelling,waist and knee pain,joint pain,dizziness and tinnitus,accompanied by skin rash,itching,aggravated condition and quickly changing condition induced by eating seafood. Conclusion Based on Delphi method,Wind Pathogen Syndrome Element Judging Expert Consensus in Chronic Kidney Diseases came into being. This scale represents most professors＇ points of view,with a high degree of authority and good coordination.
