非营养性吸允干预对低体质量新生儿的效果观察

摘　　要：目的研究分析采用非营养性吮吸干预措施对低体质量新生儿的干预效果。方法 140例低体质量新生儿采用信封法进行随机分组,试验组73例、常规组67例,两组新生儿均采用胃管肠内营养的方式,试验组加用非营养性吮吸干预措施。结果试验组新生儿的QRS波异常率(2.74%)、心率异常率(4.11%)均显著低于常规组新生儿(11.94%、17.91%),差异有统计学意义(P<0.05)。试验组新生儿的胎便排尽时间、恢复出生体质量时间、肠道营养达到481.4kJ·kg-1·d-1的时间、鼻饲管留置时间均显著低于常规组,差异均有统计学意义(P<0.05)。试验组新生儿的腹胀发生率(2.74%)、呕吐发生率(2.74%)、胃潴留发生率(5.48%)均显著低于常规组新生儿(13.43%、11.94%、25.37%),差异均有统计学意义(P<0.05);两组间呼吸暂停发生率、腹泻发生率比较差异无统计学意义(P>0.05)。结论非营养性吮吸干预措施能有效降低低体质量出生儿的心电指标异常发生率、喂养并发症等。Objective To study the effect of non-nutritional sucking intervention on low birth weight infants. Methods One hundred and forty cases of low birth weight infants were randomly divided into experimental group（n=73） and conventional group （n=67）. Both groups of infants were treated with enteral nutrition. The experimental group was treated with non-nutritive sucking interventions. Results The abnormality rate of QRS wave（2.74%） and heart rate （4. 11%） in experimental group were significantly lower than those in the conventional group （11.94%, 17.91%）, the differences were statistically significant （P〈0.05）. In the experimental group, the meconium discharge time, recovery time, nasogastric tube indwelling time and the time of intestinal nutrition reached 481.4 kJ^-1 · kg · d^-1 were significantly lower than that of the conventional group（P〈0.05）. The incidence of bloating（2.74% ）, vomiting （2.74%） and gastric retention （5.48%） in experimental group were significantly lower than than of the conventional group （13.43%, 11.94%, 25.37%）, P〈0.05. There was no significant difference in the incidence of apnea and di arrhea between the two groups （P〉0. 05）. Conclusion Non-nutritional sucking intervention can effectively reduce the incidence of ECG abnormalities and feeding complications in low birth weight infants.
