摘　　要：目的　探讨BAⅡ型国产覆膜支架治疗动物主动脉瘤的可行性、有效性和安全性。方法　用人工血管在 1 8条犬中建立腹主动脉瘤模型 ,2周后行国产镍钛记忆合金覆膜支架 (BAⅡ型 )植入术 ,术后观察 6个月 ,于术后 1个月时行计算机X射线断层造影CT检查 ,术后 6个月时处死 ,行病理学检查。结果　国产覆膜支架 (BAⅡ型 )均成功地将动脉瘤封闭 ,植入成功率为 1 0 0 %。早期的 2条分别于术后 1d和 2周时死于腹主动脉血栓 ,支架栓塞率为 1 1 .1 %。 1条于 2个月时死于内漏 ,内漏发生率为 5 .6 %。 1条于术后 1个月死于感染。其余 1 4只均无并发症发生 ,术后 6个月时的生存率为 77.8% ,临床检查示双侧股动脉搏动良好 ,病理检查示管腔均通畅 ,且覆膜内表面均已内皮化。结论　国产覆膜支架 (BAⅡ型 )在犬的腹主动脉瘤模型的治疗中是可行、有效和安全的。Objective In order to test the feasibility, compatibility and durability of covered endovascular stent graft (type BAⅡ) in the treatment of canine abdominal aneurysm model.Methods Using vascular graft, 18 canine models of abdominal aneurysm were set up. Two weeks later, self expandable stent covered with a thin polyurethane jacket ( type BAⅡ ) was used to exclude the aneurysm. All aneurysm were excluded successfully. The animals were followed up for 6 months. One month after stent graft implantation, computerized tomography was done for each dog. Six months later, all dogs were killed mercifully and pathologic examination was done.Results Two dogs died of thrombosis of the abdominal aorta at one day and two weeks after stent graft implantation with the thrombosis embolism rate being 11.1%. One died of endoleak two month after the operation with the endoleak incidence being 5.6%. Another one died of infection one month later. No complications occurred in the remaining 14 dogs. The survival rate at 6th month was 77.8%. Pathologic examination revealed that the stent graft was patent and the lumen was covered by neoendothelium.Conclusion Polyurethane covered endovascular stent graft (type BAⅡ) was feasible and effective in the treatment of canine model of abdominal aneurysm and may be used in the treatment of human aortic aneurysm.
