足月妊娠羊水偏少孕妇阴道试产88例临床观察

摘　　要：目的分析足月妊娠羊水偏少孕妇阴道试产的妊娠结局。方法对88例羊水偏少足月产妇（观察组）和同期在该院进行分娩的88例羊水指数正常足月产妇（对照组）进行研究，通过羊水指数法评估产妇羊水情况，且所有产妇均无其他的高危因素。对比两组产妇的妊娠结局。结果观察组产妇羊水量、助产时间、自然分娩率和剖宫产率分别为（308．63±148．69）mL、（25．98±7．62）h、40．91％（36／88）、52．27％（46／88）；对照组产妇羊水量、助产时间、自然分娩率和剖宫产率分别为（628．51±120．34）mL、（17．63±5．84）h、60．23％（53／88）、30．68％（27／88）；两组产妇上述指标差异均有统计学意义（t=14．43、8．16，x=6．57、8．45，均P〈0．05）。但两组产妇新生儿体质量、窒息率、胎儿窘迫率、总产程以及阴道手术助产成功率差异均无统计学意义（均P〉0．05）。结论经超声诊断羊水偏少的足月产妇，经过临床严密的监护可以进行阴道试产。Objective To analyze the effect of vaginal trial production on pregnancy outcome in pregnant women with limited amniotic fluid. Methods 88 cases with normal amniotic fluid index （AFI） were selected as control group. 88 cases with a borderline AFI were selected as observation group. AFI was used to assess maternal amniotie fluid, and all women without other risk factors. The maternal pregnancy outcome was compared between the two groups. Results The amniotic fluid volume, midwifery time, natural delivery rate and cesarean section rate of the observation group were （308.63 ± 148.69 ） mL, （25.98± 7.62 ） h, 40.91% （ 36/88 ）, 52.27% （46/88）, respectively, which of the control group were （ 628.51 ± 120.34 ） mL, （ 17.63 ±5.84 ） h, 60.23 % （ 53/88 ）, 30.68 % （27/88） ,respectively ,there were statistically significant differences between the two groups （t = 14.43,8.16, X2 = 6. 57,8.45, all P 〈 0. 05 ）. There were not statistically significant difference in the success rate of neonatal body weight, asphyxia rate, fetal distress rate, total labor rate and vaginal operation （ all P 〉 0.05 ）. Conclusion Ultrasound diagnosis of amniotic fluid less than full - term maternal, after careful clinical monitoring can be vaginal trial production.
