辅助性T细胞在小儿再生障碍性贫血发病中的表达机制及意义

摘　　要：目的 探讨辅助性Ｔ细胞在小儿再生障碍性贫血（ＡＡ）发病中的表达机制及意义。方法 将 ５３例ＡＡ患儿按病情分为重型ＡＡ组（ＳＡＡ组）２１例、轻型ＡＡ组（ＭＡＡ组）１７例、缓解期ＡＡ组（ＣＲ组） １５例，另取非ＡＡ的血液系统疾病所导致的血小板减少或者贫血患儿（ＡＬ组）１６例、健康小儿２０例（ＮＣ组） 为对照。采用流式细胞仪检测各组外周静脉血中的ＣＤ＋ ４ 、ＣＤ＋ ４ ＣＤ＋ ２５、ＣＤ＋ ４ ＣＤ＋ ２５ＣＤ１２７ｌｏｗ、ＣＤ＋ ４ ＩＦＮγ＋（Ｔｈ１细 胞）、ＣＤ＋ ４ ＩＬ４＋（Ｔｈ２细胞），采用酶联免疫吸附法（ＥＬＩＳＡ）检测外周静脉血中转化生长因子β１（ＴＧＦβ１）、 γ干扰素（ＩＦＮγ）和白细胞介素４（ＩＬ４）水平。结果 ＳＡＡ组外周静脉血中ＣＤ＋ ４ 、ＣＤ＋ ４ ＣＤ＋ ２５、ＣＤ＋ ４ ＣＤ＋ ２５ ＣＤ１２７ｌｏｗ、ＴＧＦβ１分别为（２６．５９±４．３７）％、（３．４４±０．２９）％、（３．１３±１．１６）％、（１４．５９±３．１０）ｎｇ／ｍＬ，ＭＡＡ 组分别为（３２．６７±３．１９）％、（５．４２±０．２８）％、（４．２９±１．２１）％、（２２．９８±３．３８）ｎｇ／ｍＬ，ＣＲ组分别为（３３．１３± ３．２４）％、（５．２３±０．２６）％、（４．３６±１．３３）％、（２３．１９±３．９１）ｎｇ／ｍＬ，ＡＬ组分别为（３７．９８±４．０１）％、（６．８９± ０．２８）％、（４．９９±１．４２）％、（３４．４６±５．２３）ｎｇ／ｍＬ，ＮＣ组分别为（３８．６６±３．４１）％、（７．０１±０．３８）％、（５．１０± １．５２）％、（３５．１７±５．１４）ｎｇ／ｍＬ，四Objective ToinvestigatetheexpressionmechanismandsignificaneofhelperTcellsinchildren withaplasticanemia（AA）.Methods 53childrenofAAweredividedintoheavyAAgroup（SAAgroup,21cases）, lightAAgroup（MAAgroup,17cases）,Aaremissiongroup（Crgroup,15cases）bythedisease,theothertakenthe non-aplasticanemiabloodplateletdisorderscausedbyreducedoranemiainchildren（Algroup）of16cases, 20healthychildrenwereselectedastheNCgroup.TheperipheralbloodCD＋ 4 ,CD＋ 4 CD＋ 25,CD＋ 4 CD＋ 25CD127low, CD＋ 4 IFN-γ（Th1cells）,CD＋ 4 IL-4＋（Th2cells）ineachgroupweredetectedbyflowcytometry.ELISAtestwas usedtodetectperipheralbloodlevelsoftransforminggrowthfactor-β（TGF-β）,interferon-γ（IFN-γ）and interleukin-4（IL-4）.Results TheperipheralbloodCD＋ 4 ,CD＋ 4 CD＋ 25,CD＋ 4 CD＋ 25CD127low,TGF-beta1levelsin theSAAgroupwere（26.59±4.37）%,（3.44±0.29）%,（3.13±1.16）%,（14.59±3.10）ng/mL,respectively, whichintheMAAgroupwere（32.67±3.19）%,（5.42±0.28）%,（4.29±1.21）%,（22.98±3.38）ng/mL, respectively,whichintheCRgroupwere（33.13±3.24）%,（5.23±0.26）%,（4.36±1.33）%,（23.19± 3.91）ng/mL,respectively,whichintheALgroupwere（37.98±4.01）%,（6.89±0.28）%,（4.99±1.42）%, （34.46±5.23）ng/mL,respectively,whichintheNCgroupwere（38.66±3.41）%,（7.01±0.38）%,（5.10±1.52）%, （35.17±5.14）ng/mL,respectively,thedifferencesamongthefourgroupswerestatisticallysignificant（F=23.72, 25.49,15.24,24.52,allP〈0.05）.PeripheralbloodTh1,Th2andTh1/Th2levelsintheSAAgroupwere（13.04± 中国基层医药2017年12月第24卷第24期 ChinJPrimMedPharm,December2017,Vol.24,No.24 3.01）%,（3.44±0.29）%,（1.99±1.17）,respectively,whichintheMAAgroupwere（11.01±2.89）%,（6.28± 299）%,（1.75±0.97）,respectively,whichintheCRgroupwere（10.38±3.27）%,（6.41±3.18）%,（1.62± 1.03）,respectively,whichintheALgroupwere（8.033±.42）%,（6.35±3.08）%,（1.26±1.11）,respectively, whichintheNCgroupwere（8.41±3.84）%,�
