摘 要:目的评价点阵CO_2激光联合高频电凝术治疗化脓性肉芽肿的临床疗效和安全性。方法将入选的116例化脓性肉芽肿患者随机分为两组,试验组(60例)予点阵CO_2激光联合高频电凝术治疗,对照组(56例)仅予高频电凝术治疗。每次治疗结束后3周判定疗效,并记录末次治疗后6个月内的不良反应。结果试验组1次治愈率(96.67%),明显高于对照组1次治愈率(78.57%),试验组伤口愈合时间[(7.65±1.31)d]和瘢痕发生率(3.33%)明显短于对照组伤口愈合时间[(8.74±1.48)d]和瘢痕发生率(21.43%),且以上差异均有统计学意义(P均<0.05)。试验组色素沉着发生率为6.67%,无伤口感染患者,对照组依次为14.29%和7.14%,且两组患者治疗时出血量均较少,统计学分析结果显示两组患者该3项指标的差异无统计学意义(P均>0.05)。结论点阵CO_2激光联合高频电凝术治疗化脓性肉芽肿治愈率高,伤口愈合快,治疗中出血量少,术后色素沉着、伤口感染及瘢痕发生率均较低,临床疗效及安全性均优于仅予高频电凝术治疗组。Objective To evaluate the clinical efficacy and safety of fractional CO2 laser combined with high frequency electrocoagulation in the treatment of pyogenic granuloma. Methods A total of 116 patients with pyogenic granuloma were randomly divided into two groups: treatment group(60 cases) treated by fractional CO2 laser combined with high-frequency electrocoagulation and control group (56 cases) treated singly by high-frequency electrocoagnlation. The efficacy was valued 3 weeks after each treatment and the adverse reactions were recorded in the following 6 months after the last treatment. Results The cure rate of single treatment in treatment group (96.67%) was significantly higher than that in the control group (78.57%). The wound healing time (7.65±1.31 days) in the treatment group were shorter than that in the control group (8.74±1.48 days), and the scar formation rate (3.33%) in the treatment group was lower than that in the control group (21.43%). The above differences had statistical significance(P〈0.05). In addition, the incidence of pigmentation in the treatment group was 6.7% and no infection case was found, while 14.29% & 7.14% for the same items in the control group. No obvious bleeding occurred in the both two groups. Statistical analysis showed that the differences of the above three indicators between the two groups had no statistical significance(P〉0.05). Conclusion Treating pyogenic granuloma by fractional CO2 laser combined with high frequency electrocoagulation has the following advantages: high cure rate of single treatment, quicker wound healing, less bleeding, lower incidences of pigmentation, infection and scar, meanwhile has better curative effect and safety than single high frequency electrocoagulation therapy.
