Rh血型系统抗原检测对临床安全合理输血的重要性分析

摘　　要：目的分析1003名随机抽样患者Rh血型系统抗原与输血前不规则抗体筛查鉴定结果,研究Rh血型系统抗原鉴定在临床安全输血的重要性。方法根据病史分无或单次(863例)和多次输血组(140例)。用微柱凝胶卡进行Rh血型系统抗原及不规则抗体筛查鉴定。结果患者Rh D、Rh C、Rhc、Rh E、Rhe抗原频率分别为98.80%、90.63%、59.82%、49.35%、95.71%。双群37例,无或单次输血组17例,概率为1.97%;多次输血组20例,概率为14.29%;两组双群率差异有统计学意义(P<0.05)。不规则抗体筛查阳性15例;无或单次输血组阳性2例,与Rh血型系统相关,概率为0.23%;多次输血组阳性13例,Rh血型系统抗体筛查阳性9例,概率为6.43%;两组Rh血型系统抗体阳性率差异有统计学意义(P<0.05)。结论 Rh D、Rh C、Rhc、Rh E、Rhe抗原分布频率各异,Rh血型双群与不规则抗体筛查阳性和多次Rh血型不同型血液输注有关,故输血前应对患者和献血者Rh血型抗原进行分型检测并同型输注。Objective To study the importance of Rh blood group antigen identification in clinical safe blood transfusion by analyzing the results of Rh blood type antigen and identification of irregular antibody screening before transfusion. Methods 1003 cases was divided into no or single（863 cases） and multiple transfusion groups（140 cases）. The Rh blood group antigen and irregular antibody screening were identified by microcolumn gel. Results The frequencies of Rh D,Rh C,Rhc,Rh E and Rhe antigens were 98. 80%,90. 63%,59. 82%,49. 35% and 95. 71%,respectively.（P〈0. 05）. There was significant difference between the two groups in the rate of double group（P〈0. 05）. T he positive rate of Rh blood group antibody was statistically significant in two groups. Conclusion The frequency of distribution of Rh D、Rh C、Rhc、Rh E、Rhe antigen is different,Rh blood group double group and irregular antibody screening positive in clinical caused by multiple Rh blood group different type blood infusion,so the classification of Rh blood group of the patients and blood donors was performed before clinical blood transfusion and same type infusion.
