彩色多普勒超声在泪道炎症疾病诊断中的应用价值

摘　　要：目的分析泪道炎症疾病的彩色多普勒超声(CDFI)影像特点,探讨CDFI在诊断此类疾病时的应用价值。方法泪道炎症疾病患者26例,均为单眼患病,泪小管炎患者9例,慢性泪囊炎14例,急性泪囊炎3例。应用彩色多普勒超声仪扫查泪小管、泪囊(线阵探头频率11.43 MHz),测量泪小管管径并探查血流信号,结合临床表现对超声结果进行总结分析。结果泪小管炎表现为泪管增厚、增粗、回声不均匀,前后径5.0~6.1 mm,平均5.43 mm,上下径5.1~6.3 mm,平均5.82 mm,增粗的泪小管局部彩色血流信号增多,合并泪道结石时,泪小管内见强回声区。慢性泪囊炎者泪囊壁增厚,泪囊内探及液暗区,泪囊内脓液显示为弱回声光点,泪囊壁血流信号增多。急性泪囊炎者泪囊壁增厚明显,泪囊区见囊实性不均质回声,泪囊壁彩色血流丰富。结论彩色多普勒超声作为一种无创、简捷的检查手段对正常泪小管、泪管炎症、泪囊炎症、鼻泪管不通液体积存情况显像清晰,可为临床诊断提供确切诊断依据。OBJECTIVE To analyze the characteristics of inflammatory diseases of lacrimal duct in color Doppler flow imaging（CDFI）,and to investigate the value of CDFI in the diagnosis of these diseases.METHODS Twenty-six patients with unilateral lacrimal duct inflammation,including 9 cases of dacryocanliculitis,14 of chronic dacryocystitis,and 3 of acute dacryocystitis were enrolled.The color Doppler ultrasound was used to examine lacrimal canaliculus,lacrimal sac（linear probe frequency was 11.43 MHz）.The diameter of lacrimal canaliculus was measured and the blood flow signals were detected.The clinical results were analyzed and summarized.RESULTS It showed thicken lacrimal canalicular and uneven echo in lacrimal duct inflammation.The anteroposterior diameter ranged from 5.0 to 6.1 mm,with average value of 5.43 mm,and the upper and lower diameters ranged from 5.1 to 6.3 mm,with average value of 5.82 mm.It indicated dark areas and weak echo spot in lacrimal sac of lacrimal sac in-flammation.When inflammation of lacrimal sac was acute,there was cystic echo and increased color flow signals of lacrimal sac wall.CONCLUSIONS Color Doppler ultrasound was a noninvasive and simple examination method and it yielded clear images of normal lacrimal canaliculus,lacrimal canaliculus inflammation,dacryocystitis,naso-lacrimal duct obstruction and therefore provided exact references for clinical diagnosis.
